MANUFACTURING LICENSE A. For Manufacturer a)Manufacturing License, duly renewed up to date along with list of products permitted to manufacture is to be uploaded in original, if Tenderer is a Manufacturer. b)Valid SSI/MSME or equivalent Certificate, issued by Director of Industries Commerce, Government of Karnataka/MSME or equivalent Certificate with list of products permitted to manufacture is to be uploaded in original , if Tenderer is a Manufacturer Manufacturer claimed under this Clause, otherwise they will not be treated as SSI/MSME or equivalent unit.b) For Authorized Dealers/Authorized Importers a)Notarized Manufacturing License, duly renewed up to date along with list of products permitted to manufacture for Principle Manufacturer should be uploaded, If Tenderer is an Authorized Agent/Distributor or Authorized Importer.

Authorised Agency certificate from Principal manufacturer in original format Section XIV if tenderer is an authorized agent. Note The Sub-Agents/Sub-Distributor is not eligible.

REGISTRATION CERTIFICATES UNDER COMPANIES ACT:-Registration Certificates of Companies document under Companies Act to be up loaded in original, If bidder is having company. (If applicable only)

ANNUAL TURN-OVER STATEMENT The average annual financial turnover of the tenderer should be 195.00 Lakhs during the last 3 preceding financial years (2023-24& 2024-25,2025-26) and It will be 50%i.e Rs.97 Lakhs with respect to Micro & Small Enterprises(MSME) for the last 02 preceding financial years (2024-25& 2025-26)Turnover Statement should be certified by Commercial Tax Officer or Charted Accountants to be uploaded as per Section-XVI Otherwise will be considered as Non-Responsive.

ISO:9001, CE, EC US FDA, BIS,CDSCO,AERB CERTIFICATES etc:-Standards of quality control Certificates of ISO: 9001, CE, EC,US FDA, BIS, CDSCO, AERB for model of Medical equipments/instruments/Hospital Furniture quoted only should be uploaded in original.

PROOF OF SERVICE CENTRE/S IN KARNATAKA & TECHNICAL STAFF AVAILABLE:-Service Centre/s in Karnataka with Address and Contact No & Technical Staff available for the Equipments as per the Section-XVII &Proof of Service Centre/s in Karnataka & Technical Staff available& Under taking from manufacturer for providing service for the equipment during warranty & CMC quoted under this Tender should be uploaded in original and service shall be provided within 72 hrs from the receipt of the complaint.

Performance or end user Certificate of satisfactory supply & Service from the Head of the Institute or Head of the concerned department refer Section XVI

Self declaration stating that the firm is not Black listed with any Government or Quasi Govt. Organizations and will not supply Refurbished Equipments.

Firm or Company or Organization should not be facing any judicial trial or departmental enquiry. The firm should submit a self declaration in support of this.

Technical specification, literature or information Brochure or Data sheet of the equipment.

Compatibility for the tender Specifications & Clarification for the deviations to be mentioned in the document
