Console Panel, including accessories 2 nos. Fuel Pump, 2 Nos. Diesel Tank 915 Ltr. Internal tank as per maintenance schedule, scope of work and condition attached including attending faults/ breakdown, repairing, servicing as and when required but excluding spare parts as per recommendation of OEM etc. complete as required. Installed in Regional Institute of Ophthalmology.

H – Water supply services for various buildings Centenary Super Specialty Block, Maternal and child Health Wing & Regional Institute of Ophthalmology, IMS, BHU, Varanasi (UP)

Annual Operation of water supply pump Sets for 24 hours on all working days including holidays and Sundays as required and direction of engineer in-charge. Comprehensive Annual Maintenance of electrical panels/ starter of pumps for 1 Nos. 41 HP, 1 Nos. 30 HP, 4 Nos. 10 HP, 2 Nos. 7.5 HP pumps alongwith main water pipe lines, and its associated main valves, joints etc. and pumps starter electric cables shall also be included in the scope, the spare items should be of one to one replacement type with same quality & brand The details of equipment & manpower enclosed as Annexure –A15.

Comprehensive Annual Maintenance of monoblock Centrifugal pump sets with associated electrical motors with all original spare parts including rewinding if needed the spare items should be of one to one replacement type with same quality & brand. a) 10 HP/7.5 KW Type monoblock Centrifugal pump, Make Kirloskar, Discharge type 14 LPS - 04 Nos.

Comprehensive Annual Maintenance of Submersible pumps & Motor with all original spare parts including rewinding if needed the spare items should be of one to one replacement type with same quality & brand. a) 41HP/24.6KW Type Submersible Pump, Make KSB-01 Nos. b) 30HP/22.37KW Type Submersible Pump, Make KSB-01 Nos. c) 7.5HP/5.5 KW Type Submersible Pumps Make KSB-02 Nos.

Day to day Water supply auxiliary or sub-main pipe lines, fittings, plumbing& sanitary fittings maintenance/repairing/ replacement work shall be carried out by deploying competent manpower/staff in shift of 8 hours round the clock per year, as per direction of Engineer in-charge. The contractor shall arrange all T&P, PPE and Extra Plumber (as and when required) to attend the complaints and nothing extra shall be paid on this account. The contractor shall take all precaution and measures for safety of workers while carrying out the work and nothing extra shall be paid on this account. The consumables and spares items (Fittings, fixtures and pipes etc.) for work shall be supplied by the contractor/ agency and cost for the same will be included in the scope. The materials supplied by contractor/agency should be of one to one type replacement with same quality & brand. The detail of required manpower/staff is enclosed as Annexure-A15. The works which are not included in the scope and necessary for proper functioning of the system shall also be carried out by the contractor/ agency at no extra cost.

Annual one time cleaning as & when required of all PCV/cemented roof tanks/UG tanks by manual or mechanical machines as per instructions of Engineer in-charge. Repair of the float valve leakage if any nearby the tank and on the roof and re-writing the date of cleaning, capacity of tank as already written on the tank are also in the scope. The bleaching power & broomstick shall be provided by the contractor at no extra cost. The waste materials/debris arising out of work shall be removed by the contractor from site to a place as indicated by the engineer in-charge at no extra cost. The quantity of bleaching powder should be 500 gm per 1000 Ltrs. Capacity of tank upto a maximum of 10 kgs. Note:- During cleaning of tanks, contractor should avoid wastage of water.

I- STP cum ETP services for Centenary Super Specialty Block, IMS, BHU, Varanasi (UP)

Comprehensive Annual maintenance of CCTVs installed at Centenary Super Specialty Block, Maternal and child Health Wing & Regional Institute of Ophthalmology IMS, BHU. Detailed specification is attached as per Annexure A 17

K- Complain registration and information to concerned staff

Providing services of 3 Nos. Enquiry Clerk/Telephone Operator in shift duty (6 am to 2 pm, 2 pm to 10 pm, 10 pm to 6 am one in each shift) to received all type of complaints from telephone/online. Physically and registered in complaint register and assign to concern workers/staffs/ supervisors and compliance of complaints inform the same to Junior Engineer’s/ Assistant Engineer’s on all working days including Sunday and gazette holidays at Centenary Super Specialty Block, Maternal and child Health Wing & Regional Institute of Ophthalmology contractor shall provide all kinds of communication systems to their deputed manpower/staff & to ensure proper relievers for each and every service in shifts.

All the above services & maintenance/repairing of various equipment at Centenary Super Specialty Block, Maternal and child Health Wing & Regional Institute of Ophthalmology, BHU shall be thoroughly supervised by Junior Engineers round the clock per day including holiday. Contactor shall engage junior engineer for the work as specified above. The details of Junior Engineer are enclosed as Annexure – A 18

Civil-Annual maintainance

Attending all / any type of complaints regarding Sewer Lines and Drainage in SSB.MCH, RIO by deploying Sewer man in shift of 8 hours, as per direction of Engineer-in-charge. The contractor shall arrange all normal T&P, Sewer Pump and Bamboo, Khappachi, Iron rod and Extra Sewer man (as and when required) for attending the complaints and nothing extra shall be paid on this account. The contractor shall take all precaution and measures for the safety of workers while carrying out the work. The consumable item for work shall be arranged by the contractor and nothing extra shall be paid on the account of material and T&P .

Attending all / any type of complaints regarding Carpenter work in SSB.MCH, RIO by deploying carpenter in shift of 8 hours as per direction of Engineer-in-charge. The contractor shall arrange all normal T&P for attending the complaints related to doors, window, grills , railing etc and nothing extra shall be paid on this account. The contractor shall take all precaution and measures for the safety of workers while carrying out the work and nothing extra shall be paid on this account. The consumable (fittings fixtures and wood etc. ) Item for work shall be arranged by the contractor and nothing extra shall be paid on the account of material and T&P .

Attending all / any type of complaints regarding Aluminum partition and door window work related to repair of doors, window and aluminum partition and other associated in SSB, MCH, RIO by deploying skilled staff and helpers as per direction of Engineer-in-charge. The contractor shall arrange all normal T&P for attending the complaints and nothing extra shall be paid on this account. The contractor shall take all precaution and measures for the safety of workers while carrying out the work and nothing extra shall be paid on this account. The consumable item for work shall be arranged by the contractor

Attending all / any type of complaints regarding masonry work in SSB.MCH, RIO related to repair of floors, walls, tiles flooring and cladding, granite flooring and cladding by deploying mason and helper as per direction of Engineer-in-charge. The contractor shall arrange all normal T&P for attending the complaints and nothing extra shall be paid on this account. The contractor shall take all precaution and measures for the safety of workers while carrying out the work and nothing extra shall be paid on this account. The consumable item for work shall be arranged by the contractor.

Attending all / any type of complaints in SSB.MCH, RIO regarding distempring , putty, painting etc work by deploying painters and helper as per direction of Engineer-in-charge. The contractor shall arrange all normal T&P for attending the complaints and nothing extra shall be paid on this account. The contractor shall take all precaution and measures for the safety of workers while carrying out the work and nothing extra shall be paid on this account. The consumable item (paint, distemper etc.) for work shall be arranged by the contractor. and nothing extra shall be paid on this account
