The tenderer should be a manufacturer who must have manufactured, tested and supplied the equipment (s) similar to the type specified in the ‘Schedule of Requirements’ up to at least xxxx % of the quantity required in any one of the last 3 years. The equipments offered for supply must be of the most recent series models incorporating the latest improvements in design. The models should have been released on or after xxxxx and be in satisfactory operation for xxxxx months as on date of tender opening. Certificates to be provided as per Section XII of Tender Document.
In case of authorized representative/dealer of a manufacturer - The manufacturer furnishes authorization in the prescribed format assuring full guarantee and warranty obligations as per General Conditions of Contract and Special Conditions of Contract. Manufacturers Authorization Form to be provided as per Section XIII of Tender Document.
In case of authorized representative/dealer of a manufacturer - The tenderer, as authorized representative/dealer , has supplied, installed and commissioned satisfactorily at least xxxxx % of the quantity similar to the type specified in the Schedule of Requirements in any one of the last three years which must be in satisfactory operation for at least xxxxx months on the date of tender opening. Copy of Work done and satisfaction certificate for supply of SANITORY NAPKINS Materials to the Govt / Semi Govt Institutions in the FY 2024-25. Copy of Pan Card. Income Tax return, Profit and loss, balance sheet for Last 2 Years FY 2023-24, 2024-25. Copy of Annual Turnover PER YEAR Minimum 10.00 Lakhs and above attested by CA for last 2 FINANCIAL years 2023-24 and 2024-2025. Copy of the Self declaration in Rs. 100/- bond paper should be taken in the name of District health and family welfare officer HAVERI who are submitting from Karnataka state ie The firm is not in Black listed and any other issues as per annexure-V. Bond Should be taken after tender call dated. Copy of firm prevailing trade license taken by BBMP/TMC/CMC/GP/CITY CORPORATION/HDMC/ Factories. Copy of the firm Manufactures or authorized dealer certificate. Submit the tender terms and condition copy with seal and signed in tenderers letter head. Tenderer should upload ISO certificate (Eiher bidder or manufacturer. EMD Is compulsory for all bidders and exemption for MSME with NSIC certificate holders.
