RequiredVII. Details of eligible similar nature of work(s) in respect of Structural Repair and Rehabilitation/Retrofitting with Shotcreting completed during the last seven years ending PREVIOUS DAY OF LAST DAY OF SUBMISSION OF TENDERS as per Form C . (As per performa at Page No. 83)

Required(i) The similar work means the work of Structural Repair and Rehabilitation/Retrofitting with Shotcreting work.

Required(ii) Three Similar works of minimum Rs. 11,01,208 or Two similar works each of Rs. 16,51,813 or One work of value not less than Rs. 22,02,417 during the last seven years.

Required(iii) The value of executed works shall be brought to current costing level by enhancing the actual value of work at simple rate of 7 per annum, calculated from the date of completion to the last date of submission of bid. The bidder whose performance report is good or above shall be eligible in participating the bid.

RequiredVIII. The intending bidder shall have to upload the definite proof of performance report (Experience Certificate) for the works mentioned in FORMC, on prescribed Form D issued by the appropriate authority, not below the rank of the Executive Engineer or equivalent, to the satisfaction of the competent authority. As per Performa at Page No. 84)

RequiredIX. The firm debarred by any Govt. Office will not to be eligible to participate in this bid. An undertaking to this effect shall have to be uploaded at the time of submission of online bid.

RequiredX. An undertaking to submit all original document to the O/o undersigned within 7 days from opening of bid falling which tender/bid liable to be rejected.

RequiredXI. Scanned copy of the Affidavit I/We undertake and confirm that eligible similar works (s) has/have not been got executed through another Bidder on back to back basis. Further that, if such a violation comes to the notice of Department, then I/we shall be debarred for bidding in CPWD in future for five years. Also, if such a violation comes to the notice of Department before date of start of work, the Engineer in Charge shall be free to forfeit the entire amount of Earnest Money Deposit/Perfom

RequiredXII. The enlisted contractor will not be allowed to participate in the tendering process without uploading the ERP training certificate or Sample Consent letter of Contractor for submitting e MB through CPWD ERP System. (As per performa at Page No. 22)

RequiredXIII. (a) Certificate of average Annual Financial Turnover not less than Rs. 82,591 (3 of the Estimated cost) during the immediate last five consecutive financial years duly Certified by Chartered Accountant as per Form A.

Required(b) Bankers certificate issued by any Commercial Bank of minimum Rs. 11,01,208 (40 of the Estimated cost) on prescribed FormB or Net worth Certificate of Minimum Rs. 2,75,302on prescribed FormB1 issued by Certified Chartered Accountant shall be uploaded at the time of submission of tender.

Required(C) The bidder should not have incurred any loss (profit after tax should be positive) in more than two years during available last five consecutive balance sheet (balance sheet in case of private/public limited company means its standalone financial statement and consolidated financial statement both), duly audited and certified by the Charted Accountant.
