The tenderer should be a manufacturer who must have manufactured, tested and supplied the equipment (s) similar to the type specified in the ‘Schedule of Requirements’ up to at least xxxx % of the quantity required in any one of the last 3 years. The equipments offered for supply must be of the most recent series models incorporating the latest improvements in design. The models should have been released on or after xxxxx and be in satisfactory operation for xxxxx months as on date of tender opening. Certificates to be provided as per Section XII of Tender Document.
In case of authorized representative/dealer of a manufacturer - The manufacturer furnishes authorization in the prescribed format assuring full guarantee and warranty obligations as per General Conditions of Contract and Special Conditions of Contract. Manufacturers Authorization Form to be provided as per Section XIII of Tender Document.
In case of authorized representative/dealer of a manufacturer - The tenderer, as authorized representative/dealer , has supplied, installed and commissioned satisfactorily at least xxxxx % of the quantity similar to the type specified in the Schedule of Requirements in any one of the last three years which must be in satisfactory operation for at least xxxxx months on the date of tender opening. Certificates to be provided as per Section XII of Tender Document.

Details of experience and past performance of the tenderer on equipment offered and on those of similar nature within the past three/five years2 and details of current contracts in hand and other commitments.
A detailed description of the essential technical and performance characteristics of the goods.
A list giving full particulars, including available sources and current prices, of spare parts, special tools, etc., necessary for the proper and continuing functioning of the goods for a period of two years, following commencement of the use of the goods by the Purchaser.
Satisfactory Certificate from Government, Semi Government Departments regarding Supply of SIMILAR Equipment in 03 years from the last 05 year. (Copy of the work order issued by any Government, Semi Government Departments will not be considered as part of the satisfactory certificate. Supply of consumables/reagents/drugs/furniture will not be considered as part of the satisfactory certificate.)

Bidders Should upload valid ISO 13485:2016 certificate (for medical products) issued by NABCB accreditation certification bodies. And the certificate should be verifiable online. Failure to be verified will be considered as not having NABCB certificate that is ISO 13485:2016 for medical products
The MSME & NSIC Certificate must be obtained for the supply and servicing of medical equipment, otherwise the MSME & NSIC Certificate of such institution will not be considered.
EMD Paid Details in KPPP portal or Valid MSME with NSIC/SSI Certificate issued by Government -if applicable (The bidder should submit valid MSME and also NSIC certificate issued by the Government for the supply of medical equipment or manufacturing and supply of medical equipment)
The bidder must submit a valid Registration Certificate in Form MD-42 issued by the State Licensing Authority (as per the Medical Devices Rules, 2017) to sell, stock, exhibit, or offer for sale/distribute medical devices.
Copy of GST Certificate and GST paid receipt of Any One Quarter of Last Financial Year.
Certified copy of Chartered Accountant of Annual Turnover for the past Three financial year (2021-22, 2022-2023 and 2023-24) is Minimum 03-year Turnover must be Rs 50.00 Lakh
2021-22, 2022-2023 and 2023-24 Financial Year balance sheet and Profit and loss account attested by CA
50% of the total tender value, Valid Solvency certificate from any of the national banks in the name of District Health and Family Welfare Officer Uttar Kannada and the solvency certificate should be issued between the tender starting date to the last date of uploding the tender bid. Tender number must be mentioned in the solvency certificate.
