PROCUREMENT OF EUIPMENTS FOR UHMCs

Bidder should have supplied same or similar category goods in at least one of last five years to any central/state Govt Organization/PSU/ Urban Local bodies/ Public listed company. Copies of relevant work certificates from competent authority should be uploaded. (Issued by the officer not below the rank of tender inviting authority)

Firm Name, Proprietor/Managing Director Name, Full Address and Official Mobile Number to be uploaded & (In case of Registered Partnership Firm, attested copies of partnership deed and power of attorney & Authorization to sign legal documents of tender and to submit the bid in KPPP e-Portal should be uploaded by the bidder along with the tender) In case of a company, Articles of Association, Memorandum of Association besides Resolution of BOD authorizing the Signatory to sign the bid should be uploaded).

The firm/dealer must upload a valid CDSCO license issued by the Competent Authority of CDSCO. (Central Drugs Standard Control Organization)

Bidder should upload self-declaration stating that the firm is not Blacklisted on a Rs 100 stamp paper.

The PAN Card and Goods and Service Tax Active Registration Certificate of the Tenders/Firm should be uploaded.

Balance sheet/profit & loss statement/Income tax returns statements for last 3 years (Rs. 122.00 Lakhs) (In any two of the last three Financial year) Certified by Auditor. FY (2023-24 to 2025-26) The CA certified turnover certificate with UDIN should be uploaded.
