PROVIDING D GROUP, PHARMACIST, LAB TECHNICIAN AND DATE ENTRY OPERATOR SERVICES TO GENERAL HOSPITAL AND MCH GOWRIBIDANURU FOR THE YEAR 2026-27

Bidders or organizations must submit the official Work Order and a Satisfactory Performance Certificate obtained from the concerned department for previously providing outsourcing services to a government office this includes providing a certificate issued by the head of the organization proving at least one year of experience in managing a minimum of 150 personnel during the financial years from 2023-24 to 2025-2026.

Bidders/organizations shall pay EMD Rs. 1,45,000/- through e-portal and submit the tender document

Certified copy of the certificate that the bidder/organization has registered at least 150 employees in the Personnel Department. Form-C

If the bidders organizations are individuals or organizations, the registration details and copies there of should be attached. It is mandatory that the organization should be registered in the same name for a period of 3 years i.e. from the financial years 2023-24 to 2025-2026 and the bidders should have the same PAN number in the same organization.

The bidder should submit the EPF registration certificate and EPF Paid details of last 3 financial years (2023-24, 2024-25, 2025-26) Till april 2026

The bidder should submit the ESI registration certificate and ESI Paid details of last 3 financial years (2023-24, 2024-25, 2025-26) Till april 2026

Bidders/Organizations should submit a certificate of professional tax registration and a certificate of contribution paid by the professional tax for the previous financial years 2023-24 to 2025-2026.

The bidders/Organizations should submit a certificate of registration as per the above technical condition No. 02 to 06 at least 03 years before the date of tender advertisement.

The bidders/organizations should attach audit reports (Profit & Loss A/c, Income & Expenditure and Balance sheet of for the financial years 2023-24 to 2025-2026) and the bidders/organizations should attach the details of the turnover of Rs.60 lakhs for each financial year from the financial years 2023-24 to 2025-2026 (copies attested by the CA and should have UDIN number).

Bidders must submit a valid Private Security Agency License registration certificate obtained from the Police Department. Stating no cases or enquery pending against the bidder

Provide official registration documents issued by the competent registering authority (including documents obtained from a Co-operative Society).

Submit the Form-D certificate and payment challans for 80 staff members paid to the Labor Welfare Department for the financial years 2023-24 to 2025-26.

Bidders must submit a valid Commercial/Trade License registration certificate for the year 2025-26 obtained from local bodies (BBMP/CMC/TMC/Gram Panchayat).

A valid MSME (Micro, Small, and Medium Enterprises) registration certificate must be provided.

A certification from the District Labor Department of the districts where the firm currently provides services, stating that no cases are registered against the organization, must be submitted.

A valid ISO Certification must be included.

Bidders must submit a Bank salvancy certificte for rupees 26 lakhs/- (Rupees twenty six lakhs) from a Nationalized Bank, drawn in favor of the Administrative Medical Officer, General and Mother & Child Hospital, Gauribidanur.

All copies of documents submitted with the tender must be self-attested. The bidder must sign and affix their official seal/stamp on every page of the tender documents.

The annual GST turnover must be authenticated by the CTO stating no Dues of GST for the years 2023-24, 2024-25, and 2025-26.

Organizations currently blacklisted are ineligible to participate. Bidders must submit a notarized affidavit on Rs. 200/- E-Stamp paper declaring they are not blacklisted (The bidder shall be the First Party and the District Health and Family Welfare Officer, Chikkaballapur, shall be the Second Party).

The bidders/organizations shall furnish a certificate of GST registration and a copy of the certificate of PAN card as per for the financial years 2023-24 to 2025-2026. GST 3B Clearance

The bidders/Organizations should submit a certificate of income tax paid for the previous 03 financial years i.e. 2023-24 to 2025-2026. (ITR) and PAN card should be submitted. as mentioned in the tender documents
