Requiredvi) Financial Information in Performa of Form-T2 (Certified Balance Sheet for the last three audited financials years (not to be uploaded online but to be included in physical submission only)

Requiredvii) Banker s Certificate in Performa of Form-T3A or Net Worth Certificate in Proforma of Form- T3B

Requiredviii) Applicant s Similar Work Experience (Project Sheets - One sheet for each Project) in Performa of Form-T4 ( Client certificates for work experience (for all projects as above) which shall contain information desired for evaluation of bids In case of Consultancy works of private nature other than Central/State Government/Central Autonomous Body/Central Public Sector undertaking/City Development Authority/ Municipal Corporation of city, they shall be required to submit copy of Agreement )

Requiredix) Details of Technical and Administrative Personnel to be employed for this work in Performa of Form-T5

Requiredx) Experience of Team Leader (Project Architect) in Performa of Form-T6

Requiredxi) Experience of Team Members to be deployed on the Project (one sheet for each Team Member) in Performa of Form-T7

Requiredxii) GST registration Certificate as applicable, if already obtained by the bidder. If the bidder has not obtained GST registration as applicable, then he shall scan and upload following undertaking along with other bid documents as per NIT

Requiredxiii) Under technical bid Power Point presentation of proposed project shall be uploaded

Requiredxiv) Any other information/document desired in the Bid document or relevant supplementary information/document
