RATE CONTRACT FOR SUPPLY OF BLOOD GLUCOSE MONITORING SYSTEM (GLUCOMETER & TEST STRIPS)

   1. The supply of this item is to be made only after written confirmation of demand from consignee. In quantity, as confirmed in consignee's letter, with in installment. This contract would be valid up to 3 years from the date of LOA. The requirement of total stated quantity can not be confirmed and may not be demanded for by the consignee, depending upon treating doctor reassessment.2. Price of item, if decreased during the Rate Contract period should be duly informed by the firm to PO issuing authority, so that suitable modification in purchase orders be done.3. Preference of supply should be given to this Rate contract (issued for Govt. supply), in case of shortage of goods with the company.

Standard Eligibility Criteria

   Only the self manufactured brand or manufactured by one of the division/unit of the firm is acceptable. If the brand is being manufactured by any other firm, the manufacturing firm must have been inspected by the railway and found fit by any zone of Indian Railways (except imported items). otherwise the quotation filled may be summarily rejected. Please upload the inspection document in this section.

   Photocopy of product outer paper package or/ label [which contains detailed information of product such as MRP, Manufacturing Unit, Marketing agency or firm and other relevant details such as Manufacturing dates and Expiry date] may be submitted with the tender. Please attach photocopy of product outer paper package .

   Medicine at the time of supply must have shelf life of more than 80 percent period or it should not be older than 6 months from the date of manufacture. In case of Imported medicines/special cases, where it is not possible to meet this condition, clearly mention the available shelf life of the product and if possible, attach document stating the reason(s).

   Firm must specify the Maximum Retail Price(MRP), Brand name, Name and address of the manufacturer and Distributor/Supply Point Name and address. Distributor will raise bills and payment is to be made to them online only.

   For Drugs/products of human Origin like Immunoglobulin, Human Albumin, A certification stating " Free from HIV, Hepatitis B, Hepatitis C" is required from manufacturing Firm.

   The firm certifies that the brand quoted in the tender is available in open retail market for sale by same brand name. The firm should mandatorily specify (In Remarks column) the name of drug stores along with city name, where such brand can be procured in the open market.

   Only those firm(s) are eligible to tender for a product, whose product appears in the product list of the firm uploaded on Board's website (Updated). Even if a firm is L1 but the product is not available in the list, such an offer will be summarily rejected. Repeated submission of tenders for products not included in the Firm's product list would amount to misconduct leading to punitive action against the firm. Please upload Copy of Board Approved Product List of your firm, specifying the product being quoted.

   Please enter the percentage of local content in the material being offered. Please enter 0 for fully imported items, and 100 for fully indigenous items. The definition and calculation of local content shall be in accordance with the Make in India policy as incorporated in the tender conditions.

   Code of Integrity, Misdemeanor and Penalties :- Bidders are reminded to carefully go through the IRS conditions of contract (included and uploaded along with the bid document) particularly Para 2500 pertaining to "Code of Integrity, Misdemeanor and Penalties". Bidders shall certify that none of their sister concern or affiliates (such as having common partner/director/promoter/owner/holding company decision control etc.) have participated in this tender & which can give rise to a conflict of interest or fall under anti-competitive practices.The offers of all such sister concern or affiliates if quoted are liable to be rejected as per relevant provisions of IRS terms and conditions. Bidders shall abide by the Code of Integrity in general, with specific reference to conflict of interest [Para 2502 (e)], anti competitive practices [Para 2502(c)] and obligation to proactive disclosures [Para 2503]. Bidders shall fully responsible for the declaration/certification regarding Code of Integrity, Misdemeanor and Penalties given in commercial compliance and it shall not be the Railways responsibility to verify the same. Bidders shall be liable for all consequences of violation of Code of Integrity (including termination of contract) as per provisions in Para 2505 & 2506 of IRS conditions of contract , if detected, at any stage.
