I/We, the Bidder, having read & examined in detail, the RFP document, the receipt of which is hereby duly acknowledged. I/ we, undertake that the Financial Proposal has been/shall be submitted online as per the prescribed timeline set out in the Bid document. This Proposal read with the Technical and Financial Proposal shall constitute the Proposal which shall be binding on us.

A Per bag (peritoneal bag Cost ) : The Service Provider(s) shall quote a per-bag rate inclusive of all consumables, medicines, tests, investigations, and any other (Manpower and other) costs required for the delivery of Peritoneal Dialysis services as per the scope of work. The quoted rate shall be inclusive of GST and will form the basis of the financial bid. 0 1

Note: Terms and Conditions

1 In accordance with Clause 5.1.1 (a) The Service Provider(s) shall be eligible for payment according to the financial quote (Per Bag (inclusive of all costs consumables & other) inclusive of all costs as submitted in the Financial Proposal including GST and all applicable taxes from the date of commencement of operations i.e., T0 + 60 days as provided in Clause 4.11 of this RFP document.

2 In accordance with Clause 5.1.2 (a) The Service Provider(s) shall quote a per-bag rate inclusive of all consumables, medicines, tests, investigations, and any other costs required for the delivery of Peritoneal Dialysis services as per the scope of work. The quoted rate shall be inclusive of GST and will form the basis of the financial bid.

3 The amount mentioned in the Financial Proposal shall be without any condition attached or subject to any assumption and shall be final and binding. Any tampering with the Financial Proposal format will lead to disqualification of Bidder.

4 This financial proposal must be submitted along with the financial bid, and not with the technical bid. If it is submitted with the technical bid, the bidder will be summarily disqualified from the selection process.
