Providing Annual Maintenance Contract (AMC) for HPLC Systems -40 Nos to Drugs Testing Laboratory, Palace Road Bengaluru

Proforma statement of the service orders by the service provider up to at least 80% of the scheduled requirement of the services for the AMC of HPLC Systems in any one of last three years i.e 2024-25, 2023-24 and 2022-23 in Section XII and the Performance Statement / Satisfactory Certificates of the same should be submitted Section -VIII

The bidder must upload either the Firm Registration Certificate, or a certificate of registration issued by an appropriate Government Department, such as the Company Incorporation Certificate or MSME Registration Certificate

The bidder must upload a self-declaration stating that the tenderer has not been blacklisted by any Government Department, Quasi-Government Organization, or Public Sector Undertaking, and is not facing any judicial enquiry

The bidder must upload a copy of the GST Registration Certificate along with the documents of the recent GST return paid challan for the financial year 2024-25

The bidder must upload a copy of the PAN Card.

The Declaration of Acceptance of Scope of Services (Section VI) must be duly filled, signed, and uploaded by the tenderer

The Tender Bid Offer Form (Section XIV) must be duly filled, signed, and uploaded by the tenderer.

Bidder should submit Authorization letter from Original Equipment Manufacturer (OEM) or should submit the trained technician declaration trained by Original Equipment Manufacturer (OEM) / Authorized Service Provider to provide service shall be in the format provided as per Section -XIII or Section -XIII A whichever is applicable

Income tax returns statements for the last three financial years i.e. 2022-23, 2023-24 2024-25 should be uploaded (income tax return acknowledgement copies).

The average annual financial turnover of the tenderer should be minimum of rupees 20.00 Lakhs (it will be 50% with respect to Micro & Small Enterprises) for each of the three financial years i.e. 2022-23, 2023-24 & 2024-25 (last 2 preceding financial years with respect to Micro & Small Enterprises) and certified by a chartered accountant, copy of the annual turnover declaration by a chartered accountant should be submitted.
