Information regarding experience in operation and maintenance of Acrylic Throwball Court Arena during the last three financial years (2023-24, 2024-25 , 2025-26). Experience certificates obtained from the owners or competent authorities of Acrylic Throwball Court practice facilities, or documents confirming ownership, operation, and maintenance, shall be enclosed.

Experience certificate for running a Acrylic Throwball academy issued by a registered club of the Karnataka State Throwball Association (Regd.) shall be submitted.

Recent 2 passport-size photographs of the authorized signatory of the tenderer/organization

An affidavit on Rs.500/- stamp paper confirming that the tenderer has not been blacklisted by any authority during the last 05 years shall be submitted.
