RequiredBidder should submit Authorization letter from Original Equipment Manufacturer OEM or should submit the trained technician declaration trained by Original Equipment Manufacturer OEM Authorized Service Provider to provide service shall be in the format provided as per Section XIII or Section XIII A whichever is applicable.

RequiredIncome tax returns statements for the last three financial years 2022-23, 2023-24 2024-25 should be uploaded income tax return acknowledgement copies.

RequiredProforma statement of the service orders by the service provider up to at least 80 percent for the primary manufacturer and up to at least 30 percent for the authorized representative agent of a manufacturer of the scheduled requirement of the services for the AMC of HPLC Systems in any one of last three years 2024-25, 2023-24 and 2022-23 in Section XII and the Performance Statement Satisfactory Certificates of the same should be submitted Section VIII.

OptionalSmall Scale Industries Certificate

RequiredThe average annual financial turnover of the tenderer should be minimum of rupees 04.00 Lakhs it will be 50 percent with respect to Micro and Small Enterprises for each of the three financial years 2022-23, 2023-24 & 2024-25 last 2 preceding financial years with respect to Micro & Small Enterprises and certified by a chartered accountant, copy of the annual turnover declaration by a chartered accountant should be submitted.

RequiredThe bidder must upload a copy of the GST Registration Certificate along with the documents of the recent GST return paid challan

RequiredThe bidder must upload a copy of the PAN Card.

RequiredThe bidder must upload a self-declaration stating that the tenderer has not been blacklisted by any Government Department, Quasi-Government Organization, or Public Sector Undertaking, and is not facing any judicial enquiry

RequiredThe bidder must upload either the Firm Registration Certificate, or a certificate of registration issued by an appropriate Government Department, such as the Company Incorporation Certificate or MSME Registration Certificate

RequiredThe Declaration of Acceptance of Scope of Services Section VI must be duly filled, signed, and uploaded by the tenderer

RequiredThe Tender Bid Offer Form Section XIV must be duly filled, signed, and uploaded by the tenderer.

Eligibility Criteria

Bidders shall not be under a declaration of ineligibility for corrupt and fraudulent practices issued by Government of Karnataka

It shall be the responsibility of the Bidder to ensure credit of Tender Processing Fee and EMD into the respective receiving bank accounts of e-Procurement on or before the last date and time of bid submission.

EMD Payments through e-Payment mode shall be made as one single transaction and payments made in part are liable for rejection.
