AMC/CAMC of PSA Medical Oxygen Generator & Air Compressor of PLW Railway Hospital Patiala for two years.

The bidder shall be submit along with the bid documents, documents in support of their claim to fulfill the minimum eligibility criteria as mentioned in the tender document. Bidder shall submit a certificate regarding truthfulness of documents submitted by firm(as per Performa attached) If any documents (or copy thereof) submitted by a bidder is found to be false/forged. The bidder/each partner /member for the bidding firm shall be liable to punitive actions ,as decided by competent authority of railways. In such an eventuality, the bid shall also be summarily rejected. If the contract has already been awarded, letter of acceptance (LOA) has been issued and in the event of any failure to comply with the above, the contractor shall terminated, irrespective of the stage of progress in execution of the work in such an eventuality EMD/ security deposit (SD) performance guarantee (PG) and partial / full payment otherwise due to the contractor in respect of the partial /full work executed by the contractor shall be forfeited by the Railways. Other punitive action ,like banning the bidder and partner /member of the bidding firm for further dealing with Indian railways and /or the government of India may also be taken by the railway /Govt. of India of authorities.

I/we hereby declare that I/we have downloaded the tender documents from Indian Railway website www.ireps.gov.in . I/we have verified the content of the document from the website and there is no addition, no deletion or no alteration to the content of the tender document. In case of any discrepancy noticed at any stage i.e. evaluation of tenders, execution of work or final payment of the contract, the master copy available with the railway Administration shall be final and binding upon me/us.

I/we declare and certify that I/we have not made any misleading or false representation in the forms, statements and attachments in proof of the qualification requirements.

I/We also understand that my/our offer will be evaluated based on the documents/credentials submitted along with the offer and same shall be binding upon me/us.

I/we certify that I/we the tenderer(s) is/are not blacklisted or debarred by Railways or any other Ministry / Department of Govt. of India from participation in tender on the date of submission of bids, either in individual capacity or as a HUF/ member of the partnership firm/LLP/JV/Society/Trust.

I/we understand that if the contents of the certificate submitted by us are found to be forged/false at any time during process for evaluation of tenders, it shall lead to forfeiture of the Bid Security and may also lead to any other action provided in the contract including banning of business for a period of upto two year. 1.Vendor shall maintain adequate spare parts (brand/make of the compatible with current plant) for smooth running of the p l a n t . 2.The scope of work also includes unconditional AMC/CAMC of installed oxygen plant. 3. Vendor should ensure that the PSA oxygen plant (including the O2 plant-related problems) on any working day is set right within 24 hours of reporting the complaints & in no case, later than two working days. 4. The vendor shall maintain the equipment as per the manufacturer's guidelines and shall use standard and genuine components for replacements. 5. Complaint can be registered either telephonically or by email or any other way as deemed fit by respective office and a proper record of the complaints to be maintained by the AMC/CAMC vendor. 6.The engineers deployed for PLW Hospital Patiala will get signed in visit reports registers from PLW Hospital Patiala. 7.The vendor (AMC/CAMC) shall carry out preventive maintenance (PM) on quarterly basis and shall plan, as per schedule of quantities, such as maintenance is carried out in each equipment at least once in three months. A separate logbook should be maintained to record the PM carried out in each equipment.
