Attach a letter of confirmation that the Human Resource Agency has provided the services of at least 05 staff members including 02 government departments/organizations for the year 2024-25 and that the said departments/organizations have provided satisfactory services.

Attach a copy of the service organization name, address, and mobile number on the letterhead.

The service organization must have a Form-C registration certificate with the Labor Department, which must have at least 5 employees registered and must be renewed for the current year.

GST Registration Certificate and GST Tax Returns (December-2025 to February -2026).

Registration certificate under EPF Act and copies of deduction challans (December-2025 to February -2026).

Registration certificate under ESI Act and copies of deduction challans (December-2025 to February -2026).

A declaration stating that the candidate is not an elected member/the organization is not blacklisted and that there are no inquiries in any department at the time of submission of tender should be submitted on a notarized paper of Rs. 100/- as per Annexure-3 issued by this office.
