Provident fund ( PF) Registration Certificate : ( Attach valid Registration Certificate of agency obtained from the issuing authorities)

Satisfactory completed, in at least one financial year in last five years 2021-22 to 2025-26, one similar contract of Providing D-group/Dalayaths and Data Entry Operator Services of value not less than Rs. 24.85 lakhs . ( (Attach Experience Certificate issued by the Employer of State Government/ Central Government /State or Central undertaking boards / Urban local bodies )

Satisfactory completed, in at least one financial year in last five years 2021-22 to 2025-26, Providing Services of total D-group- 10, and Data Entry Operator- 07 (Attach Experience Certificate issued by the Employer of State Government / Central Government /State or Central undertaking boards / Urban local bodies )

Any 12 months Provident fund ( PF)remittance challans D-group -10, and DEO- 07 in last 12 months i.e April 2025 to March 2026: ( Attach minimum 12 months Provident fund ( PF) remittance challans)

ESI department Registration Certificate : ( Attach valid Registration Certificate of agency obtained from the issuing authorities)

Any 12 months ESI remittance challans of D-group/ 10, and DEO-07 in last 12 months i.e., April 2025 to March 2026( Attach minimum Six months ESI remittance challans)

GST/Service Tax Registration Certificate : ( Attach valid Registration Certificate of agency obtained from the issuing authorities)

Self declaration by the agency stating that the agency has not involved in blacklist in the stamp paper of Rs.500.

Achieved in at least two financial years in last five years 2021-22 to 2025-26 annual financial turnover of Rs 49.69 lakhs (Attach Certified balance sheet issued by Charted accountant (With profit loss statement)
