ANNUAL TURN-OVER STATEMENT:- The average annual financial turnover of the tenderer should be Rs.15.00 Lakhs(estimated cost of tender) It will be 50% (Rs.7.50 Lakhs) of the estimated cost of the quantity with respect to Micro & Small Enterprises) during the last 3 preceding financial years (2022-23, 2023-24 & 2024-25) (last 2 preceding financial years (2023-24 & 2024-25)with respect to Micro & Small Enterprises) ending on the relevant financial year

PAN CARD:-PAN Card of the Firm/Tenderer is to be uploaded in original.

GST REGISTRATION CERTIFICATE:-GST Registration Certificate (Form REG-06) of the Tenderer in original is to be uploaded.

TENDER OFFER FORM-PART I:-Tender Offer Form with an undertaking to abide by the terms and conditions of the tender in the format as per Section-IX-Part-I, of the Tenderer is to be uploaded in original.

DECLARATION FROM-PART II:-Declaration from the Tenderer in the format as per Section IX-Part-II of the Tenderer is to be uploaded in original.

LIST OF ITEMS QUOTED UNDER THIS TENDER:- List of Equipments/Instruments along with Manufacturer/s Name, Country of origin and Model quoted by the tenderer should be clearly furnished separately as per Section-XVIII, is to be uploaded in original.

PROOF OF SERVICE CENTRE/S IN KARNATAKA & TECHNICAL STAFF AVAILABLE:-Service Centre/s in Karnataka with Address and Contact No & Technical Staff available for the Equipments as per the Section-XVII &Proof of Service Centre/s in Karnataka & Technical Staff available& Under taking from manufacturer for providing service for the equipment during warranty & CMC quoted under this Tender should be uploaded in original and service shall be provided within 72 hrs from the receipt of the complaint.

FORMAT FOR LIST OF DOCUMENTS/INFORMATION SUBMITTED UNDER THIS TENDER:-SECTION-XIX-Check List. The Check list must be submitted in PDF Format & MS Word Format with filling of all information without fail, otherwise treated as Techno-Commercially Non-Responsive

EMD PAID DETAILS: EMD Payment details copy should be uploaded. EMD Exemption: Uploading MSME Registration with NSIC Certificate and as per the NSIC condition the EMD will be exempted otherwise it will not be exempted

Bidder will need to inspect site and provide necessary planning, drawings, civil, electrical, plumbing and partition BOQ as per site requirements and need to get approved from The Director, Koppal Institute of Medical Sciences, Koppal before uploading in the tender, if fails such tenderers tender will not consider

Bidder should have valid Electrical License & completed at least 1 similar turn key projects in Govt of Karnataka
