A The Supplier shall submit the earnest money deposit certificate.

B The supplier shall submit the certificates of GST and PAN

C Identity card (driving licence or Aadhaar card or passport copy)

D Signed photograph of the tenderer.

E The supplier shall submit the certificate mentioning the name of bankers.

F The supplier shall submit the constitution of the firm (whether the tenderer is a proprietorship/ partnership or Company etc.)

G The supplier shall submit the duly signed General power of attorney given by the company to sign papers and documents etc.

H Tender should be a service provider/agency in Disposal of plastic waste processing unit. In case of disposal of plastic waste through plastic processing unit, whether the Tenderer submitted the copy of valid Consent to Operate (CTO) and EXTENDED PRODUCER RESPONSIBILITY (EPR) Registration Certificate along with offer. (Submission of offer without CTO &EPR Certificate will not be considered.

I Tenderer should have an experience of Minimum one (1) year in assisting the customer to get registration with central pollution control (CPCB) including collection and compilation of all the required documents to form an Action Plan. Documentary proof should be submitted along with the Tender.

J Weather the tenderer provided the list of urban local bodies (ULBs) to whom they have agreement for plastic waste disposal along with the copies of valid agreement/documents with ULB’s in respective states.

K Number of ULB’s/ number of states

L Number of disposal facilities/units

M The Supplier shall submit the undertaking to the effect that the tenderer have no dispute with Markfed and are not black listed by any of the agency.
