Satisfactory certificate of at least 3 years of experience in similar work in the Health Department ,Government/Semi-Government Organization in the past (2022-23, 2023-24 & 2024-25

Self-declaration letter stating that you are not blacklisted by any government/semi-government organization. (Rs. 500 should be printed on a notarized paper).

Submitting the ISO (9001:2015) certificate obtained for providing human resource services

Attach the Karnataka Professional Tax Registration Certificate valid for the current year and the return form for 2024-25.

Must be GST registered, have clearance from the authorized authority for the current year from April-2025 to January-2026, and must have paid GST from a single authorized number

A copy of the organization's PAN card must be attached and income tax must be paid. Income tax payment records for the last 3 financial years must be provided (2022-23, 2023-24 & 2024-25).

Annual turnover (of Mana Power Agency only) for the last 3 years certified by a Chartered Accountant for each year (2022-23, 2023-24 and2024-25) Certificate and Balance Sheet to be attached.

Attaching a Solvency Certificate of Rs. 92,55,006/- (Rs. Ninety-two Lakh Fifty-five Thousand and Six Rupees only) obtained from a nationalized bank.
