Submission of Covering letter as required in the given format (Refer Appendix-1)

Furnishing of the Power of Attorney Power of Attorney executed by the Bidder in favour of the Principal Officer or duly Authorized Representative, certifying him as an authorized signatory for the purpose of this Tender

The bidder should have registered with ESI, PF, Labour departments /authorities in the state of Karnataka Copy of registration certificate issued by these respective departments/authorities

Bidder should obtain latest Income Tax clearance certificate from I.T Department or IT returns Copy of the latest Income Tax clearance certificate from I.T Department or IT returns

Bidder should hold Valid GST certificate and Permanent Account Number (PAN) issued by income tax department.Copy of valid GST certificate and Permanent Account Number (PAN) issues by income tax department should be provided (Refer Appendix-4)

The bidder should have an annual turnover of at least Rs 3.00 Crores each year of last 3 years Certified copy from Charted Accountant shall be provided as per the format given in Appendix 2

The bidder should have deployed manpower for services for any Public, Govt. institutions worth 75 Lakhs at least in one of the last 3 years and should at least have 5 years experience in the concerned field. Copies of relevant Work Orders/ Purchase Order

Submission of last 3 years ESI/EPF paid challans Copies of ESI& EPF paid challans for last 3 years

The bidder should have an office in Bengaluru, Karnataka Undertaking from the Company Secretary or the Authorized Signatory as per Appendix-3
