Experience certificate for having executed similar type of service in any Govt., Medical Institution or Govt., Hospital by engaging minimum 100 workers.

Self declaration certificate to effect that firm is not black listed & not in default in respect of any previous Institution where the service is already provided. The declaration to be executed on stamp paper worth Rs.500/-. In the name of (first party agency second party in the name of tender inviting party)

Certificate to the effect that the firm is not involved in any criminal case/criminal activities to be obtained from concerned District Police Officer.

Solvency certificate of Rs.100.00 lakhs from the Nationalized bank issued after the notification of tender, in the name of Director, KMCRI Hubli

The institution has invited tenders for different packages. The tenderer is eligible to be allotted only one package among the total packages. The tenderer shall execute declaration in stamp paper worth Rs.500/- agreeing the above clause.

If more than one tenderer quotes the same rate tender may be decided by lottery system will be implimented. The final decision vests with Director, KMC&RI Hubli. The tenderer should give a declaration in the stamp paper of Rs.500/- agreeing the same.

Non refundable tender form fee of Rs.20,000/- shall be paid in the form of DD favoring Director, KMCRI Hubli before last day of submission of tender. Scan copy of DD acknowledgement shall be uploaded in the portal
