Manufacturing License a).Manufacturing License, duly renewed up to date along with list of products permitted is to be uploaded in original, if Tenderer is a Manufacturer. If Tenderer is an Authorized Agent/Distributor should upload the Notarized copy of Manufacturers Manufacturing License along with registration & other relevant documents registered branch in India.b).Registration (if bidder is a Tenderer) by registration of Companies document to be up loaded in original.

Authorization letter from the Original Equipment Manufacturer (single authorization is allowed per OEM).

The tender document shall have Seal and Signature on all pages including the Annexure by the authorized signatory

Demand Draft Rs. 5000/- DD favoring AMO, KIMS Hospital & Research Centre Payable at Bangalore

The equipment quoted should have the approval of all the competent authorities and should have F.D.A or C.E or I.S.O as per the concern Department Equipment Specification provided by HOD.

Statement of deviations parameter wise from tendered technical specificationscommercial conditions, if any.

Service center in Bangalore & Technical staff available.

GST clearance certificate for the last three financial years or GSTR 3B return of each month of last 3 months.

Technical specification and Information brochure of the Equipment/ instruments in original.

ANNUAL TURNOVER CERTIFICATE: -Bidder shallhaveaminimum averageturnoverofRs.25croresfromthelast3 Financial years ending 31st March 2026 and

ITR RETURNS:-The Tenderer should have Income Tax Returns ITR for the last financial years 2023-24, 2024-25 and 2025-26 duly certified by Charted Accountant and should upload in original.
