Past experience certificate for supply of similar items of the type specified in the Schedule of Requirements is 80% ie Rs.8.46 lakhs in any one of the last 5 years ie 2021-22 TO 2025-26 Work done certificate should be issued from not below the rank of Executive Engineer or Equivalent
The tenderer should furnish the information on all past supplies and satisfactory performance in proforma under Section XII
The bidder / OEM should not have been Delisted/ barred/penalized by the Government for the reasons of not fulfilling or breach of terms of the Portal as on the date of Tender submission.
Supplier should be OEM/authorized dealer.

For women supplying of Sari with blouse for women PKS. Material -polyester, Length-not less than 6.5m, GSM-60-70, Colour Khaki/blue. Along with Cotton Polyester Medical Staff type Aprons with 40(+/-5) GSM. Apron to be labelled in embroidery as per the instructions. (The pair includes one sari with blouse and one Apron). Supplied to the different ULB locations in the district.
For men Supplying of long sleeve shirt and Trouser with polyester blend suiting for uniforms, having minimum 67 percent polyester (+/-3%) and remainder Cotton/viscose conformity to IS 11248,1995 with latest updates. Colour Khaki/blue. And finished product to be labelled in embroidery as per the instructions. (The pair includes one shirt and one trouser) Sizes- small, medium, XL, XXL and XXXL. Supplied to the different ULB locations in the district.
