RequiredAnnexure-VI (Affidavit-III) stating that the tenderer or his representatives are not having any links with the Rice Millers/Mill Lorry Owners Associations/Foodgrains wholesale.

RequiredAnnexure-VII (Affidavit-IV) for stating that they have not declared the same stage-II vehicles for another MLS point within district or the MLS points of neighboring districts for obtaining stage -II contracts

RequiredAnnexure-VIII (Affidavit-V) for stating that no forgery of signatures and fabricated documents submitted for the purpose of award of contract.

RequiredAnnexure-IX for Lorry Owner Associations as indicated in clause no 9(iii) of tender terms and conditions shall be uploaded and furnished along with hard copies.

RequiredCopies of Adhar, GSTIN & PAN should be uploaded.
