RequiredKMC Registration certificate of the Gynecologist Doctors. Self-Declaration of the Gynecologist Doctors confirming the association with the Bidder.

RequiredRegistration/ Qualification certificate of the qualified 10 Women Nursing Staff.

RequiredThe Bidder shall furnish the details of the Organization.

RequiredThe bidder should be a Health Care Organization for a minimum period of 2 years.

RequiredThe Bidder should be registered as a legal entity such as company registered under Companies Act 1956/2013 or Societies Registration Act or proprietorship or Partnership or LLP or an equivalent law applicable in the state for a minimum of 2 years as on date of bid submission.

Eligibility Criteria

Bidders shall not be under a declaration of ineligibility for corrupt and fraudulent practices issued by Government of Karnataka

It shall be the responsibility of the Bidder to ensure credit of Tender Processing Fee and EMD into the respective receiving bank accounts of e-Procurement on or before the last date and time of bid submission.

EMD Payments through e-Payment mode shall be made as one single transaction and payments made in part are liable for rejection.

The bidder should have carried out individually or in association with any NGO/ CSR Activities related to Menstrual Hygiene Training Services in any State Govt. / Central Govt. Departments/ Boards/ Govt. Organizations/ Public Sector Units in the last 3 financial years (FY 2021-2022, 2022-2023 & 2023- 24).
