Name and address of the tenderer

Details of registration with the Department of Labor (A copy of the registration certificate must be attached and a certificate from the Department of Labor stating that the conditions have not been violated must be submitted)

Details of the code number obtained under the EPF Act (copy attached) and a certificate of payment for the year 2025-2026 obtained from the Commissioner.

Details of the code number obtained under the ESI Act (copy to be attached) and a certificate from the Deputy Director of ESI regarding payment for the year 2025-2026.

Attach the professional tax registration details and the payment slip (2025-26) of professional tax paid for 1 year to the concerned department, and submit Form-05 duly certified.

GST (copy of registration letter should be attached) and information on the amount paid by the organization in the year 2025-2026 including office details should be attached, and TDS details deducted from the offices every month and confirmation letters regarding the reconciliation of the amount paid in 3B with the challan paid should be submitted.

Submit a letter confirming the payment of income tax for the last 2024-25 and 2025-26 years to the Income Tax Department in the prescribed format.
