Form C Certificate from labour dept. The Firm should enclose the current year Certificate. As per labour Act 1970.

GST Registration Certificate. If exempted from GST attach certificate

Enclose PAN card ,ESI,EPF,PT Copy

Balance Sheet for the financial year 2021-22, 2022-23 and2023-2024, There should not be any negative Capital or Negative Balance in the any of the Financial Statements.

Attach A declaration In Rs. 200/- Bond paper by Notary. If the disbursal of Amount is delayed by Government, the firm should certify that they will Run Mobile Medical Unit with their own cost.

Attach A declaration In Bond paper by Notary, that the firm is not Blacklisted, Defaulter in any of the Government, Semi Government Department. (Rs.100/- Bond paper)

Income Tax clearance certificate for the Assessment year 2022-23 and 2023-24 & 2024-2025
