Five Lakh Only) and buffering of Rs. 20,00,000-00(Rs. Twenty Lakh only)

Eligibility Criteria

Bidders shall not be under a declaration of ineligibility for corrupt and fraudulent practices issued by Government of Karnataka

It shall be the responsubility of the bidder to ensure credit of Tender Processing Fee and EMD into the respective receiving bank accounts of e-Procurement on or before the last date and time of bid submission.

The bidder is required to ensure browser compatibility of the computer well in advance to the last date and time for receipt of tenders. The department shall not be responsible for non-accessibility of e-procurement portal due to internet connectivity issues and technical glitches at bidders end.

EMD Payments through e-Payment mode shall be made as one single transaction and payments made in part are liable for rejection.

The Bidder should follow the terms and conditions of the Bank.

The Bank has the right to accept, reject or cancel the tender.

Family Floater-Employee,Spouse, Dependents, Children and Parents.

Cover for pre existing diseases from day one, Internal and External shall be covered.

Maternity extension normal-Rs. 25,000-00 and Maternity extension Caesarean-Rs. 40,000-00

Waiver of 9 months waiting period for maternity (from day one cover)

Emergency Ambulance charges up to sum of Rs. 1000-00 per hospitalisation and overall limit of Rs. 2000-00 per policy period.

Sub limits only for Cataract Rs. 50000-00 per person per policy period.

TPA and Broker Companies are not allowed.

Cash Less treatment in PAN India and all day care procedures covered.

Bidder Health Insurance Company shall have branch at Hassan, Karnataka.

Pre Hospitalisation 30 days and post Hospitalisation 60 days applicable.

Corporate Buffer of Rs. 20,00,000-00 (Rs. Twenty Lakh only) to be extended after 5.00 lakh Sum assured completed.

All other terms and conditions as per SGHI policy clause.

Advanced modern treatments shall be covered.

Up to this date, claim of existing policy is Rs. 89,76,800-00 and remaining policy period is up to 14-11-2023.
