The Bidder shall have a minimum of two years of experience in carrying out similar type of assignment/service in any State Government / Government of India / Public Sector Undertaking/private sector, A Satisfactory Completion Certificate issued by the end users.( 23-24 & 24-25)

EMD amount of Rs.70950 to be submitted in Karnataka Public Procurement Portal only.

Standalone Company or Consortium / The applicable consortium partner submitting the Proposal should upload a Certificate for having registered with the Labor Department in concerned state .(Shops and Establishment ACT)

The bidder has NABL and any other accreditation registered documents should attach. (National Accreditation Board for Testing and Calibration Laboratories is crucial for laboratories).

Biomedical Waste Management (CTF) Certificate with the appropriate authority valid as on date of submission of tender documents.

The Bidder shall have operated & Managed diagnostic laboratory services, as per table in pre-qualification criteria. Should have valid KPME Registration.

Standalone Company or Consortium/ Lead Bidder/ Partner submitting the Proposal should have 1 registered MD Pathologist Specialist on its payroll as administrative /operations role for the proceeding 1 year (Agreement).

Affidavit as Self Declaration for Non-Black Listed attested from Notary (on Rs. 100 stamp Paper) in favor of District Health & Family welfare Society, Kolar district (Latest copy from the date of tender publication).

Standalone Company or Consortium/ All the Individual partners of the consortium submitting the Proposal should have GST Registration and should have filed GST Returns of financial year 22-23, 23-24 and 24-25.

Standalone Company or Consortium/ Lead Bidder/ Partner submitting the Proposal should have filed their IT Returns for the last 21-22,22-23 & 23-24 Financial Years. Copies of ITR returns should be submitted in the proposal & Copy of PAN Card.

Standalone Company or Consortium/ partner/ Lead Bidder submitting the Proposal should upload audited Balance Sheet / Profit & Loss Statement and also audit report (Form 3CA,3CB,3CD) for the last 21-22,22-23 & 23-24 Financial Years certified by a Chartered Accountant with his / her name, signature, registration number, UDIN NUMBER, contact phone number and address.
