Tender for Providing under NHM out sourcing Manpower Services 2026-27 to District Health and Family Welfare Office Chikkabalapura

Submit work orders and satisfactory performance certificates issued by relevant departments for outsourcing services previously provided to government bodies. Experience certificates must cover the financial years 2022-23, 2024-25, and 2025-26.

The bidder/firm must submit proof of payment of EMD of rupees 3,33,590/- paid through the e-Portal.

The bidder/firm can be an individual or an entity. If it is an entity, registration details along with copies must be attached. The firm must have been registered under a single name continuously for a minimum of 5 financial years-specifically 2021-22, 2022-23, 2024-25, and 2025-26-and the bidder must possess a single PAN number registered under the same firm name

Certified copy of the registration certificate from the Labour Department showing a minimum of 400 registered employees.

EPF registration certificate and copies of contribution payment challans for the financial years 2022-23, 2024-25, and 2025-26.

ESI registration certificate and copies of contribution payment challans for the financial years 2022-23, 2024-25, and 2025-26.

Professional Tax registration certificate and proof of PT clearance for payments made during the financial years 2022-23, 2024-25, and 2025-26.

Documentary evidence proving that the registration certificates requested in technical conditions Sl. No. 02 to 06 were issued at least 04 years prior to the date of this tender notification.

Income Tax Returns (ITR-5) Submission of Income Tax Returns filed for 05 financial years (Previous Year 2021-22, 2022-23, 2023-24, 2023-24, 2024-25) (Assessment Year 2022-23, 2023-24, 2024-25, 2025-26)

Submit 4 years of Audit Reports (Profit & Loss A/c, Income & Expenditure, and Balance Sheet) for the financial years 2022-23, 2023-24, 2024-25 and 2025-26 Submit Chartered Accountant (CA) certified turnover details of at least 1.50 Crore (One Crore Fifty Lakhs) per annum for each of the financial years 2022-23, 2023-24, 2024-25 and 2025-26 complete with a valid UDIN number.

Registration certificate/license to run a private security agency obtained from the Police Department.

Supporting documents (RTGS, NEFT details, and official cover letter) proving that wages were paid on time to employees deployed previously for the past 3 months.

Submit Form-D challans showing contribution payments for 100 staff members to the Labour Welfare Department for the years 2023, 2024, and 2025.

A valid Police Verification Certificate issued by the District Police Department confirming that no criminal cases are registered or pending investigation against the bidder/firm. Mandatory to submit.

Trade/Commercial License registration certificate for the year 2025-26 obtained from local authorities (BBMP / CMC / TMC / Gram Panchayat).

A clearance certificate from the District Labour Department stating that no cases are pending against the firm (to be produced at the time of issuing the work order).

A Bankers Cheque for 3,000,000/- (thirty Lakhs) drawn from a nationalized bank in favor of the District Health and Family Welfare Society, Chikkaballapur.

Copy of a valid license under the Contract Labour (Regulation and Abolition) Act, 1970.

Bank Guarantee (BG) for rupees 20,85,000/- equivalent to 5% security deposit, obtained from a nationalized bank in favor of the District Health and Family Welfare Society, Chikkaballapur (to be submitted at the time of issuing the work order).

All copies of documents submitted with the tender application must be self-attested with the tenderers signature and official seal on every page.

Blacklisted firms are strictly ineligible to participate. A non-blacklisting affidavit notarized on a rupees 500/- e-stamp paper must be submitted, where the District Health and Family Welfare Officer, Chikkaballapur is named as the First Party and the participating tenderer/firm as the Second Party.

GST registration certificate along with proof of GST clearance or copies of GSTR-3B returns for the financial years 2023-24, 2024-25, and 2025-26. A copy of the PAN card must also be provided.

GST turnover must be authenticated by the GST Department for the financial years 2021-22, 2022-23, 2024-25, and 2025-26, exceeding rupees 2,00,00,000/- (Two Crores) per annum
