The agency should have experience of at least 3 years providing outsource man power. 3 years of Experience Certificate issued by the head of the Department indicating provided Man Power services to the Government/ Semi Government Departments along with work satisfaction certificate (2023-24, 2024-25 and 2025-26)

The Agency should have been registered at least 3 years before, with the Labour Act. (Form-C)

E.P.FRegistration Code number and challans of successful payment of EPF for the year 2023-24, 2024-25 and 2025-26

ESI Registration Certificate from E.S.I Deputy Director and challans regarding successful payment of ESI for the year 2023-24, 2024-25 and 2025-26

The Agency should not have been blacklisted by any State Government / Central Government and / or any State / Central PSU. A notarized declaration to this effect should be furnished on stamp paper of Rs.100/-

Tenderer must comply in office letter head to supply man power as per recent Government of Karnataka order providing reservations in outsource employees

GST Registration and GST (Service Tax) returns filed during the last 1year i.e (2025-26) and GST paid challan for the month of April-2026

Income Tax returns filed during the year2023-24, 2024-25 and 2025-26

Professional Tax Registration Certificate and copies of Professional Tax filed during the year 2023-24, 2024-25 and 2025-26

The Agency must furnish 3 years of Audit Report (Audited by CA) with UDIN number

Payment details towards Karnataka Labour Welfare Fund for the year 2025 and 2026. FORM D
