Certificate of experience in providing human resource services to a 150 bed hospital and original copies of office orders should be attached.

Registration Certificate of the Labor Department Form-C and its updated copy should be attached.

A copy of the challan of at least 150 employees paid in each year for the Labor Welfare Fund should be attached. A copy of Form-D and copies of DD or Net Bank receipts should be attached (for the financial year: 2022-23, 2023-24 and 2024-25) (should be before the tender is called.)

Employees Provident Fund (EPF) Registration Certificate and copies of the departmental challans paid monthly for the last 03 years should be attached. (For the financial year: 2022-23, 2023-24 and 2024-25)

State Employment Insurance (ESI) Registration Certificate and copies of the monthly challans paid for the last 03 years should be attached. (Financial Year: 2022-23, 2023-24 and 2024-25)

PAN Card should be attached.

Professional Tax Registration Certificate of the company, copies of annual and monthly payment of contributions of the employer and employee for the last 3 years should be attached and the copies of the payment of the challans should be prior to the call for tenders. (Financial Year: 2022-23, 2023-24 and 2024-25)

The organization should attach copies of income tax returns filed for the last 3 years (Financial Year: 2022-23, 2023-24 and 2024-25)

To show financial strength, Rs. 3234551.00 lakh original certificate (Solvency) in the name of District Surgeon, Public Hospital, K.G.F. Details obtained from Nationalized Bank (Original letter to be attached)

The tenderer should attach the C.A. of the last 03 years. Attach a copy of the audit report obtained from them and the average turnover certificate of the last 3 years of Rs. 03 crore (Financial Year: 2022-23, 2023-24 and 2024-25)

The tenderer should submit a certified undertaking in the name of the District Surgeon, Public Hospital, K.G.F. on a stamped paper of Rs. 200/- in the name of the District Surgeon, Public Hospital, K.G.F.

Appendix-1 should be filled, signed and submitted.
