Required(v) Profit Loss statement of the last three years ending on 31-03-2022 certified by CA as per Form A.

Required(vi) Details of eligible similar nature of works completed during the last seven years ending as per Form B.

Required(vii) Details of works under execution or awarded during the last seven years ending as per Form C.

Required(viii) List of consultancy work of projects abandoned during last seven years in Form D.

Required(ix) Performance Certificate of work experience as per Form E.

Required(x) Details of Structure and organization as per Form F.

Required(xi) Details of technical and administrative personnel in Form G.

Required(xiv) GST registration Certificate of State in which the work is to be taken up if already obtained by the bidder. If the bidder has not obtained GST registration in the State in which the work is to be taken up or as required by GST authorities then in such a case the bidder shall scan and upload following under taking along with other bid documents. If work is awarded to me I we shall obtain GST Registration Certificate of the State in which work is to be taken up within one month etc

Required(xvi) Affidavit to engage associate sub-consultants having domain specialization as per 1 e above.

Required(xvii) Scanned copy of PAN card of company issued by Income Tax Department copy of registration of company other document to show minimum experience of 10 Years Any one document.

Required(xviii) Affidavit that bidder will have his own office in Delhi NCR as per 1 f in prescribed format I We hereby under take that my company has registered office in Delhi NCR or I shall have office in Delhi NCR during the period of planning Execution consultancy of work.
