RequiredDeclaration to the extent that, the sample submitted to TTD is one and the same that was tested in any one of the labs of Central Silk Boards /TEXTILE COMMITTEE (as mentioned in the annexure)

RequiredThe samples should carry Silk mark label/tag

RequiredThe firm should have a turnover of more than Rs.50.00 lakhs for the preceding one financial year and should furnish a certificate certified by Charted Accountant to that effect

RequiredThe relevant particulars should be furnished in the prescribed proforma only
