Attach a copy of the service experience certificate obtained from the concerned department, certifying the satisfactory provision of outsourced human resource services comprising a minimum of 50 personnel for a period of one year to a Central/State Government department, a semi-government organization, or an autonomous government institution during the years 2024-25 and 2025-26

Attach proof of payment of the EMD amount of Rs. 143411/- (calculated at 2% of the tender value) made via the e-portal.

Bidders must clearly state and attach their name and the full postal address of their organization (including mobile number, office telephone number, and e-mail address) on their agency/organization's letter head.

The organization shall attach Annexure-1 in respect of sole proprietorship or partnership.

Attach the existing registration certificate (Form C) issued by the Labour Department of the Government of Karnataka regarding the registration of the outsourcing agency.

Attach a copy of the GST registration

Attach the ESI registration certificate.

GST payment challans for 6 months (January 2026 to June 2026) for the 2025-26 financial year.

Challans for ESI contribution payments made during the 2025-26 financial year (for the six months from January 2026 to June 2026)

PT Clearance Certificate regarding payment of Profession Tax

Please attach copies of the Indian Income Tax Return acknowledgments for the three financial years: 2023-24, 2024-25 and 2025-26

Please attach the annual turnover certificate for the last three financial years (2023-24, 2024-25 and 2025-26), certified by a Chartered Accountant.

As per Annexure-2, the outsourcing agency must attach a self-declaration (notarized) on a stamp paper worth Rs. 100/- (or the currently applicable value) stating that it has not been blacklisted during the years 2023-24, 2024-25 and 2025-26
