RequiredAudited Balance sheet and Profit & Loss account and ITR Acknowledgements of the Applicant for each of the last 3 audited financial years FY 2021-2022, 2022-2023 & 2023-24. CA Certificate for Annual turnover for last three financial years with UDIN No.

RequiredThe Bidder shall furnish the details of the Organization.

RequiredThe Bidder should be registered as a legal entity such as company registered under Companies Act 1956/2013 or Societies Registration Act or proprietorship or Partnership or LLP or an equivalent law applicable in the state for a minimum of 2 years as on date of bid submission.

RequiredThe Bidder should have their Own Bio-Medical Waste Management System or shall have tie-up with Authorized Bio-Medical Waste Management System.

RequiredWork Completion/ Satisfactory Certificate and its relevant Work Order issued by Competent Authority to be submitted.

Eligibility Criteria

Bidders shall not be under a declaration of ineligibility for corrupt and fraudulent practices issued by Government of Karnataka

It shall be the responsibility of the Bidder to ensure credit of Tender Processing Fee and EMD into the respective receiving bank accounts of e-Procurement on or before the last date and time of bid submission.

EMD Payments through e-Payment mode shall be made as one single transaction and payments made in part are liable for rejection.

Technical Criteria

Past Experience

The bidder shall have carried out a Preventive Health Checkup Services (Cumulative Work Done Value) in any State Govt. / Central Govt. Departments/ Boards/ Govt. Organizations/ Public Sector Units in the last 3 years (FY 2021-2022, 2022-2023 & 2023-24).

Work Done Certificate

The bidder should have executed Preventive Healthcare Checkup Centers with a minimum value of Rs. 5.00 Crores in a single work order for any State Govt/ Central Govt Departments/ Boards/ Govt Organizations/ Public Sectors in the last 3 years (FY 2021-2022, 2022-2023 & 2023-24).

The bidder should have performed Preventive Healthcare Checkups for any State Govt/ Central Govt Departments/ Boards/ Govt Organizations/ Public Sectors in the last 3 years (FY 2021-2022, 2022-2023 & 2023-24).

Capabilities of Vendor

The bidder should be registered with ESI & EPF as on date of tender submission.

Others -- Relevant documents to be uploaded.

The bidder should be mandatorily registered with KPME Level-2 or equivalent certificate issued by concerned state Health Department registered for a minimum of 2 years as on date of bid submission.

Methodology and Approach for overall implementation of projects. A presentation in the form of colour printout should be submitted along with the bid.

Financial Status

The bidder shall have a minimum Average Financial turnover of Rs. 15.00 crores in last three financial years duly Audited or attested by CA with UDIN No. (FY 2021-2022, 2022-2023 & 2023-24).
