E-tender for contracting non-clinical and cleaning services for Genral Hospital, Shidlaghatta

Bidders who have previously provided outsourcing services to any government department must submit, Work order copies and Performance certificates issued by the concerned departments for the financial years 2022-23, 2023-24, and 2024-25. A self-declaration certificate listing the government departments served year-wise with human resource services.

Bidders/organizations to submit a document showing that they have paid EMD of Rs. 1,20,000/- through the e-portal.

Bidders / Institutions must be either an individual or a registered institution. If it is an institution, registration details and copies of relevant certificates must be attached.It is mandatory that the institution must have been operating under the same name for at least 5 years, i.e., during the financial years 2019-20, 2020-21, 2021-22, 2022-23, and 2023-24, and the bidder must possess a single PAN number registered under the same institution name.

Certified copy of the certificate from the Labour Department showing registration of at least 100 employees (Form-C).

EPF (Employees Provident Fund) Registration Certificate, and copies of challans evidencing EPF contributions for the financial years 2021-22, 2022-23, and 2023-24, as well as for March 2025.

ESI (Employees State Insurance) Registration Certificate, and copies of challans evidencing ESI contributions for the financial years 2021-22, 2022-23, and 2023-24, as well as for March 2025.

Professional Tax Registration Certificate, and proof of Professional Tax payments for the financial years 2021-22, 2022-23, and 2023-24, as well as for March 2025.

For the technical criteria mentioned in serial numbers 02 to 06 above, the bidder must submit documentary proof that the respective registrations were obtained at least 5 years prior to the tender publication date.

Bidders must provide salary payment records for the last 3 months, as per statutory compliance (RTGS, NEFT and Office letters), confirming that employees appointed through their organization were paid salaries on time.

Submit Labour Welfare Fund (Form-D) challans showing contributions made for at least 100 employees for the years 2022,2023,2024

A valid Police Verification Certificate (Character Verification Certificate) issued by the District Police Department, clearly stating that There are no pending criminal cases or No investigations are pending against the bidder or the institution. This certificate is mandatory.

Trade/Commercial License Registration Certificate for the financial year 2025-26, issued by a local body such as BBMP / CMC / TMC / Gram Panchayat.

MSME (Udyam) and ISO Certification must be submitted.

A certificate from the District Labour Department of every district where the bidder is providing services, clearly stating that no legal cases have been filed against the institution. The certificate must be for the financial year 2025-26.

A Bankers Cheque for 2,50,000 (Rupees Two Lakhs fifty thousend) must be submitted in favor of Medical Superintendent, Government Hospital, Sidlaghatta, drawn from a nationalized bank.

All documents submitted along with the tender application must be self-attested. The tenderer must affix their company seal and initial every document submitted.

Blacklisted firms/institutions are not eligible to participate in this tender. A notarized declaration (on Rs 100 e stamp paper) must be submitted, confirming that the bidder is not blacklisted. Bidders/organizations are required to submit a certificate of GST registration and a copy of the GST certificate attached to it for the financial years 2023-2024 and 2024-25 up to April-2025 and a copy of Schedule-3 and provide a copy of PAN card.

Bidders / Institutions must submit the following documents and proofs Income Tax Return Acknowledgments (ITR-V) for three financial years 2021-22 (Assessment Year 2022-23) 2022-23 (Assessment Year 2023-24) 2023-24 (Assessment Year 2024-25) Along with a copy of the PAN card.

Audited financial statements including Profit & Loss account, Income & Expenditure statement, and Balance Sheet for the financial years 2022-23 2023-24 2024-25 These reports must be attached.

Bidders must also submit proof of a minimum turnover of 1.00 crore for each of the above three financial years, certified by a Chartered Accountant, and must possess a valid UDYAM registration number.
