Providing service of vehicle with driver for AYUSH DEPT KOLAR

Information about providing vehicles to any government/semi-government:

public enterprises in the last two years and a certified copy of the service certificate obtained from the office.

Tours and Travels registered in the State of Karnataka Name and full address of the agency/organization and PAN number (Attach a photocopy of the registration certificate)

Copy of payment of EMD money in e-procurement

Attach copies of vehicle registration number, RC book, current insurance and emission certificates

Attach a copy of the vehicle documents along with the vehicle documents of the driver.

Permanent residence of the driver Telephone / Mobile number.

A copy of the e-stamp paper certified by a notary that the tour and travel agency has not been blacklisted in government/semi-government/public enterprises (after the date of issuance of the tender notification).

A document stating that the tour and travel agency has two vehicles

Registered Tours and Travels Agency/Organization Related to the financial years 2023-24 and 2024-25 Attach a copy of the income tax paid documents

Registered Tours and Travels Agency/Organization Submit P&L Account and Balance Sheets for the financial years 2023-24 and 2024-25, certified by the Auditor.

Attach copies of GST Registration Certificate and GST (Audit) of the registered Tours and Travels Agency/Organization for the month of April-26.
