To show financial strength, original solvency certificate of Rs. 10,00,000 (Ten lakh rupees only) in the name of District Health and Family Welfare Association Kolar, a copy of which has been obtained from a nationalized bank

Registration number and K.A. certificate of 15 vehicles registered in 2020 or above, fitness certificate, H-Electronic Fuel Consumption Certificate, Pollution Control Certificate of vehicles and driving license of 15 current vehicle drivers (2 vehicles should be in the ownership of the bidder, the remaining vehicles should be provided by the owner. If the said bidder is selected as the first bidder, the said vehicles will be provided by the owner, and the contract copies will be provided during the technical verification)

Current Form-C obtained from the Labor Department should be attached.

Copies of all documents submitted for tender should be self-attested. Tenderers should affix the seal of the organization and sign all the tender documents.

Pan card and income tax returns filed for the previous 03 years are to be submitted year-wise as follows. (Assessment year 2023-24, 2024-25 & 2025-26)

Audit Reports (Profit & Ioss Balance Sheet details and copies attested by them) for the last 03 (2023-24, 2024-25 & 2025-26) years, totaling Rs. 1.50 crore in the last three years, along with copies attested by them.
