Registration Certificate issued by the Municipal Authority or any other Authority established by the Govt. of India or Karnataka mentioning the business as Food Catering Services/Hotel/Hospitality Services Business

G.S.T. Registration certificate

Food License & Registration Certificate issued by Food Safety & Standards Authority of India under FSS Act 2006 and amendment thereon

Registration/License certificate from the Labour and Employment Department mentioning the business as Food Catering Services/Hotel/Hospitality Services Business

Registration/License certificate from Employees Provident Fund Organization

Registration/License certificate from Employees State Insurance Corporation

PAN card of Firm/Agency/Trust

Declaration on stamp paper that Firm/Agency/Trust has not been Blacklisted by any Govt/Non-Govt department and the owner/partner of the firm/agency/trust is not involved in any police/judicial case.

Declaration on stamp paper regarding Number of Staff deploying for the purpose of catering services during the training program:(1) Cook staff (2) Helpers, (3) Service staff, (4) Cleaners/Housekeeping staff (5) Manager. (Break-up Details for TWO SHIFTS- Morning to Afternoon & Afternoon to Night should be provided).

Certificate regarding clearance of all statutory deductions, viz, EPF, ESIC and salary/wages to staff of your Firm/Agency/Trust.

Declaration on stamp paper regarding Hot & Cold Utensils available for the food services.

Satisfactory Certificate from the present office/organization where the bidder is providing catering service to be obtained and attached. THIS IS MANDATORY REQUIREMENT. In case this certificate is not attached the technical bid of the bidder will be rejected.

Annual Turnover certificate for the Financial Year 2023-24, 2024-25 & 2025-26 duly authenticated by the Chartered Accountant.

Copy of Income Tax Returns filed for the Financial Year 2023-24, 2024-25 & 2025-26.
