Note - The entire work mentioned in item No 01 to 05, including comprehensive, preventive and periodical maintenance of lifts / elevators shell be got executed through OEM i.e. M/s OTIS Elevator company (India) Ltd only. The contractor will submit the MOU with M/S OTIS Elevator company within 15 days of AOC. (ii) Grade of oil required in gear box shall be India oil, Servo Mesh 320 pr Servo system 320 equivalant Grade of oil all as approved by Engg - in - charge & deemed to be included in the quoted rates. (iv) Any items not covered shall be deemed to be included in the quoted rates for the smooth fuctioning of lifts and contractor may quote the rate accordingly. contractor is expected to inspect the lift before quoting the rates.

(v) Contrator shall be attending complaint round the clock i.e 24 hrs. If contractor is not able to rectify the complaint, penalty shall be charged as given below (a) upto 1st day- NIL (b) 2nd day- Rs 1000/- (c) Subsequently for every day- Rs 1200/day (vi) Responce time shall be 3 working hours if any breakdown call is received during late hours the same will be attended by early next morning. The call back service will be provided with respect of WEF 9.00am to 5.30 pm in case of any emergency the service may be provided beyond this timing. As against major breakdown the time needed for repair/replacement of the following items to the extent indicated should also not to be counted as down time. a. rewinding/replacement of main motors 07 days b. Warm Gear 07 days. c. Speed governor rope 02 days d. Brake shoe liner 01 day e. Transformer Rectifier 05 days f. Tralling cable replacement 05 days g. Brake Magnet /Door drive motor rewinding alcement 05 Days

Outsourcing or services of sewage treatment plant by providing skilled operator man for operation of STP of capacity 150 cum/day and all associated connected accessories for a period of 11 months at Military Hospital Bareilly to keep installation in functional condition in two shifts per day (each shift 8 hrs) i.e. from 0600 to 1400 hrs and 1400 hrs to 2200hrs including maintaining log sheets and other periodical maitenance record complete all as specified and directed

Contractor shall keep the following aspects in mind while quoting unit rates and rates quoted shall be deemed to be involving the following: -

STP/ Lift operator should be holding ITI certificate by authorised technical board and experience certificate have to be submitted by the contracotr by dully verified and attested for operating pump DG set and sub station where dispute exists the operators qualification/ performance will be assessed by Engineer-in-incharge. Decision shall be binding on contractor. FGM shall posses, police verification certificate.

FGM should not be more than 55 years of age and should not be deriving pension from any Govt/semi Govt or public sector undertaking.Any disputes will be at discretion of GE and decision shall be binding on contractor

Necessary security passes shall be arranged by the supervisor in respect of all personel deployed as per prevaling station rule and regulation.

Tools/plant and saftey equipments required to complete the minor work in sluice valve/pie lines/LT panels shall be provided by the contractor.

Operator shall posses, police verification certificate.

Maintain the internal and external area of plant.

Watch and ward and care of Govt Assests and equipments of the plant

Maintain the safety and security measures of the plant

keep the installation neat and tidy

Disposal of treated sewage/sludge as directed by the Engr-in-Charge.

Efficient treatment of the soil water shall be ensured by the contractor and required charactersitics of outlet treated sewage shall be as under:-

(a) Oil and grease < 10g/lit

(b) Suspended solids < 100 mg/Lit

(c ) BOD < 20 mg/lit

(d) COD < 100 mg/lit

(e) Coliform count 100 count per 100ml

The operator deployed by the tenderer shall also be responsible for supply of treated water from plants to various distributary lines for arboriculture including running of pump etc.

Contractor shall ensure through operator for day to day upkeep and check of pump motor set as per manufacturers instructions and carry out minor repairs of following nature by own arrangement to the satisfaction and direction of Engr in Charge :-

(a) Glands

(b) Studs

(c ) Greasing

(d) Dusting of plant & machinery.

(e) Weeding/ rooting out grass roots, vegetation etc.

Note for item No 7 to 10 :- Contractor shall liable to provide necessary reliver of each lift operator/STP Operato duing absence and leave period of any Lift operator/STP Operator. The same shall be intimated timely to Engr-in-Charge. Lift operator/STP Operator shall carryout Manual operation as per duty roster in each working day including Sunday & Holiday. Lift Operator and STP operator must wear uniform provided by the Contractor. The unit rate quoted shall be inclusive of payment of prevailing minimum fair wages as per govt circular and as applicable including payment of ESI, EPF and GST all as applicable. In case the required manpower as given in Sch'A' notes is not engaged, necessary recovery to this effect based on double the quoted rate will be recovered from the contractor. Payment to employed persons strictly to be done directly to AADHAR linked bank account only. The contractor shall submit all documents pertaining to EPF, ESI, bank statements of the employed persons along with RAR claims; Without these mandatory documents, No claim shall be made and the contractor shall have no claim in this regards.

Checking, testing and replacing of unsv parts as required of 63 mm hydrant valves and making hydrant valve functional in each aspect complete all as specified and directed Note:- (i) Spare parts shall be measured and paid separately here-in-after from item no 1.01 to 1.04 as supply only (ii) The Cost of any spares such as nut/bolts,hose reel drum , jubli clip ,gasket etc. not mentioned here-in-after in the item deemed to be included in quoted rate

Note :- (i) All defective parts will be replaced by contractor within 48 hrs as per above mentioned fire fighting and fire alarm system. (ii) In order to certify proper maintenance of the system, monthly visit of Engineer from authorised rep of company to check the efficient running of fire fighting system including all acceseries of alarm system is mandatory on part of contractor. The unit rates quoted by the tender shall be deemded to include this aspects. Necessary documents in support of deployment and visit of authorised representative from compony shall be submitted by the contractor to Engr-in-Charge and his comments obtained in writing before making any payment to contractor after each visit. On failure of visit by auth. representative or non submission of report by him, a panel recovery of Rs. 5000/- per visit shall be made from the contractor dues.
