Company Registration Organization should register in any of the governmentdepartment like DIC/Municipality/Registrar of companies and organization should have the registered office in Karnataka

The Agency or Tenderer should have PAN CARD and should be uploaded in original

GOVERNMENT OF KARNATAKA DEPARTMENT OF LABOUR REGISTRATION CERTIFICATE OF ESTABLISHMENT FORM-C (See Rule 4):-The Agency should have valid Registration Certificate of Establishment issued under Karnataka Shops and Commercial Establishment act of 1961, according to Government of Karnataka, Department of Labour or its equivalent and should have registered with minimum 10 (Ten) Labors engaged should be uploaded in original.

CERTIFICATE:-The Agency should have GST REG-06 registration certificate and should be uploaded in original.

GSTR Returns Statement for the last 06 Month in original should be uploaded.

CERTIFICATE:-The Agency should have valid EPF registration certificate issued by the Office of the Regional Provident Fund Commissioner as per EPF Act and should be uploaded in original.

The Agency should submit the EPF Remittance Statement its equivalent minimum for Rs.01.00 Lakhs per Annum for having remitted for the Financial year 2021-22 to 2025-26 duly certified by the Office of the Regional Provident Fund Commissioner or competent authority as per EPF Act should be uploaded in original.

The Agency must have valid ESI registration certificate issued by the Office of Sub-Regional, ESIC or competent authority of the Department as per ESI Act and should be uploaded in original.

The Agency should submit the ESI Remittance Statement its equivalent minimum for Rs.01.00 Lakhs per Annum for having remitted for the Financial year 2021-22, 2022-23, 2023-24, 2024-25 and 2025-26 duly certified by the Office of Sub-Regional, ESIC or competent authority should be uploaded in original.

The Agency should submit the Annual Turnover for the last Five financial ears (2021-22, 2022-23, 2023-24,2024-25 and 2025-26) i.e., for having at least two financial years an average minimum financial turnover of Rs.50.00 Lakhs as per ANNEXURE1

Income Tax Returns ITR for the last three Financial years 2021-22, 2022-23, 2023-24, 2024-25 and 2025-26 in original should be uploaded duly certified by the Tenderer.

The Agency should have valid ISO certificate (related Erection, commissioning and maintenance from last 5 years) should be uploaded in original

The Agency should have at least two similar contracts having minimum capacity of 500 KLD STP at Government Hospitals/Government Autonomous Medical Institutions for last 03 financial years of 2021-22, 2022-23, 2023-24 2024-25 &a 2025-26 the details as per the Performance Contracts tatement as per the ANNEXURE-II along with EXPERIENCE CERTIFICATES as per ANNEXURE-III or its equivalent or as per the Principal Employers Format that consisting of required information and its related WORK DONE should be ploaded in original

The Agency shall give Self Declaration in Rs.500/-Stamp Paper attested by the Government Notary as per the ANNEXURE-IV 1 stParty-Tenderer 2 nd Party-Director, KMCRI, HUBBALLI

The Tender Offer Form as per the ANNEXURE V should be uploaded in original.

Contractors should upload their office and home address along with telephone numbers in their companys letter pad
