FORM FIN-4 BREAKDOWN OF REIMBURSABLE EXPENSES When used for Lump-Sum contract assignment, information to be provided in this Form shall only be used to demonstrate the basis for calculation of the Contract ceiling amount, to calculate applicable taxes at contract negotiations and, if needed, to establish payments to the Consultant for possible additional services requested by the Client. This form shall not be used as a basis for payments under Lump-Sum contracts

Addition / Deduction

The cost of enterprise grade solution - including Licensing Costs, Integration Costs, Cloud Storage cost, ATS (Annual Technical Support) cost and other related expenses for a period of 7 years (5 years of the contract period plus an additional 2 years post-contract completion).

Cost for Handling Change Requests (Blended Person months Rate for 100 Person-months) This payment will be made on pro-rata person-month basis only if the client approves such change request / service request.

Note: 1. “Per diem allowance” is paid for each night the expert is required by the Contract to be away from his/her usual place of residence. Client can set up a ceiling.
