1. Medical Superintendent, GMERS ,GENERAL HOSPITAL, GANDHINAGAR invites on line tender

for Work of Reagent Rental for Fully Automated Clinical Biochemistry/Chemistry analyzer for various

Test”. Tenderer are requested to sign & stamp all the pages of tender form, documents, Which are to

be submitted physically (hard copy), otherwise tender will be rejected.

2. Demand Draft of Rs.5000/- (Rupees One Thousand five Hundred only) as amount of Tender Form

Fees & Demand Draft of Rs.3,00,000/- (Rupees Threer Lakh only) as Earnest Money Deposit

(EMD) for said work in favor of, Medical Superintendent, GMERS ,GENERAL HOSPITAL,

GANDHINAGAR issued by any Nationalized Bank payable at Gandhinagar (Gujatrat) should be

3. EMD will be returned to the tenderer if it‘s tender is not accepted, But if the tender is accepted, it

a. If the tender is accepted but the tenderer fails to execute agreement or pay security deposit

than E.M.D. will be forfeited.

b. Incomplete tenders & tender without earnest money deposit will not be taken into

General Hospital, Gandhinagar Delivery basis.

5. IN NO CASE, RATES SHOULD BE QUOTED ANYWHERE EXCEPT IN ON-LINECOMMERCIAL BID THE

6. Tenderer should submit the attested true copy / notarized copy of all required and mandatory

documents in hard copy which are to be submitted with Technical Bid physically:

a. Firm Registration Documents- in case of small scale industries than SSI Certificate b). Sales Tax

/VAT Registration/GST Registration.

7. Risk purchase clause: If the tenderer fails to do the work as per this office Order within

delivery period of the tender, The Medical Superintendent, GMERS ,General Hospital ,Gandhinagar

will do the work from elsewhere or from suitable Sources on account & at your risk. The tenderer

shall be penalized to the extent of difference of rates or a 10 % of the value of earlier orders

whichever is higher.

8. Delayed payment shall not be considered for the excuse for late or delay work/non supply etc.

9. Any type of recovery will be made from tenderer under Government rules and regulations and land

11. After submission of tender, no tender related inquiry in person would be entertained and the

successful bidder would be intimated through email. If any such activity is found during the tender

a. Validity of Tender: - Validity period of the tender will be 180days from the date of

opening of Technical Bid.

b. CONTRACT PERIOD: - The contract for said work will be 60 Months from the date of

issuance of contract. However, Medical Superintendent, GMERS General Hospital,

Gandhinagar reserves the right to extend the validity of the contract, if required.

13. Medical Superintendent, GMERS General Hospital, Gandhinagar reserves all rights of canceling the

tender completely/partially any time during validity period without assigning any reason thereof.

14. If any dispute or difference arises in the present subject matter of the case between Tenderer and

GMERS, General Hospital, Gandhinagar the same will be subject to arbitration.

15. Tenderer has to abide by all statutory Rules, Regulations set by appropriate authority.

16. The firm quoting for the tender should be primary manufacturer for quoted products. The

authorized dealer quoting the tender must submit. The Valid authorization certificate from the

primary manufacturer.

(B) GENERAL TERMS & CONDITIONS

1. Tenderer must attach copy of Fresh authority letters (After the date of Tender Notice) from

manufacturer in prescribed format as per Annexure -I If authority letter from mfg. Company of

which rate quoted is not attached in Technical Bid, the rate for that particular item/work

will be rejected.

2 .The envelope which should be properly sealed and superscripted as "Technical Bid for

3. In Absence of Any Documents required in technical bid, particular tender in question will be

6. Commercial Bid Submission :

6.1 The commercial bid should be submitted on line on or before the date and time Stipulated in

schedule of e- Tender at the website only on https://www.nprocure.com.

6.2 Rates SHOULD NOT BE quoted anywhere else except in prescribed format given online,

otherwise the tender liable for disqualification and will be rejected without any consideration.

6.3.Tenderer must quote the rates of items in column :4 “RATE AS PER COMMERCIAL BID WITH

ALL TAXES” mentioned in our commercial bid column : 3. Tenderer can mentioned their own

pack size in “REMARKS” column : 6. But the price must be quote as per our offered unit for easy

7. OPENING OF TENDER:-

In the first instance, only "Technical Bid" will be opened on the date of opening of the Technical

bid and will be taken into consideration for finalization.

7.1 Opening of Technical Bid Documents will be commence on Date & Time shown in tender

schedule at the office of Medical Superintendent, GMERS ,General Hospital, Gandhinagar.

7.2. Tenderer or representative authorized by him/her, can remain present at the time of tender

opening. Only one person will be allowed.

7.3. After evaluation of Technical Bid, Commercial bid of only those bidders will be opened online

whose Technical Bid fulfill all the requisite terms and conditions of the tender and qualified

from technical bid opening.

7.4. Technical Scrutiny of items where specification mentioned in commercial bid of tender form in

column:2,will be done by a committee of experts nominated by the Medical Superintendent,

GMERS ,General Hospital ,Gandhinagar or concerned Head of the Department & his/her decision

about the technical acceptability is final. Commercial bid will not be opened in case of items

technically rejected by basis of your original Literature/catalogue / brochures of items where

Technical Scrutiny will done on the specification mentioned in tender form by concerned Head

of the Department.

7. Back out from tender at any interim level during tender processing : Once the tender is submitted

it will be the responsibility of the tenderer not to escape halfway directly or indirectly by way of

raising any problems.

9. Rate should be quoted FOR General Hospital , Gandhinagar and Inclusive of all Tax.

9.1 Tenders are invited from the Manufacturers/Authorized Firms/Suppliers. Technical Bid should be

Submitted with all documents/information. Separate sheet can be used in case of more space is

required, and it should be attached with technical bid. The tender will be rejected, if tenderer does

not fulfill the criteria of technical bid.

9.2 Tenderer should submit EMD by a Demand Draft Payable at Gandhinagar . This amount will be

treated as security Deposit or submit the require amount , in case the tender is accepted .

Demand Draft for EMD should be submitted with TECHNICAL BID, otherwise tender will be

9.3 (a)Supply of the drugs/items must be as per Pharmacopeia stander / FDCA approved/require

Specifications. (if applicable).

(b) The goods having near expiry will not be accepted. If materials are declared "substandard" by

Drug Authority or concern authority not approved by purchase committee members, then

the supplier will have to make full replacement by other.

9.4 Said tender will be operated by this office in absence of contract fixed by Gujarat Medical

Services Corporation Ltd, Gandhinagar (G.M.S.C.) If G.M.S.C.L. contract is done after issue of

order from this office the Medical Superintendent will have all the right to cancel the said order

at any time. No communication will be considered in this regard.

9.5 In the event of breakages or loss of stores during transit against requisition order the said

quantity/equipment/items /reagents will have to be replaced by the tender holder. The purchaser

will not pay separately for transit insurance and the supplier will be responsible for the stores.

In the event of breakages orloss of stores stores received but not later than 15 days from the

date of arrival of stores at destinations during transit it should be informed to the supplier as

soon as possible.

10. Delivery Period: Delivery Period: The maximum delivery period will be three Months from the

date of receipt of purchase Order. However you have to supply immediately if require.

11. Late Supply Penalty Clause: Tender fail to deliver the store of any part there of , within contract

period of delivery three weeks or in case the store are found not in accordance with the

prescribed specification or approved sample, Medical superintendent, GMERS, General Hospital,

Gandhinar, shall exercise his discretionary power either, as applicable.

(i) To recover from the tender as liquidated damages a sum not exceeding half a percent(1/2%) of

the price of the stores which you have failed to delivered aforesaid each week or part thereof

during which the delivery of such stores may in arrears but subject to maximum limit in the

case of an order not exceeding Rs. One lack Rs.1,00,000/-) in value of 10% (ten percentage) and

in a case of an order exceeding Rs.1,00,000/- in value of 5%(five percent)of the stipulated price

of the stores undelivered.

(ii) To cancel the contract.

12. IMPORTANT NOTE: RISK PURCHASE CLAUSE : if the tenderer / supplier fail to supply the goods as

per this office order within delivery period of the tender or not as per specifications/ standerd , the

work order will be cancelled straightway without any notice and order will be place to the firm who

will supply/do the concern work the material as ready stock or from open market and difference of

rate or 10% (ten percent) of the value of earlier order whichever is higher from their E.M.D. and if the

authority permits for late delivery than half a percent per week penalty will be recovered from the bill

. Acceptance of goods/work after delivery period will be at discretion of Medical Superintendent

GMERS, General Hospital, Gandhinagar.

13. The Risk Purchase will be done at any time after the delivery period is over from open market

or L- 2 firm on non supply of goods or not providing the said work by the defaulter.

14. Replacement Clause: If any stores supplied against our Order are found to be of not of standard

quality on inspection and / or analysis by the competent authority, the contractor shall be liable to

replace the entire quantity of the relevant order. The decision of the Medical Superintendent,

GMERS, General Hospital, Gandhinagar taken on the basis of the report of the competent laboratory

regarding goods not of standard quality will be final and binding. If the replacement or payment as

specified is not made by the contractor, the procedure for risk

purchase as shown above will be initiated without any further reference or intimation to the

15. The Tender can also be terminated by the Medical Superintendent, GMERS, General Hospital,

Gandhinagar in the following circumstances:

1. If the firm is debarred or disqualified or ceases to exist or convicted of any offence.

2. If the quality of the item to be supplied is found not up to the standard and multiple samples

are found to be not of standard quality.

1. TECHNICAL SPECIFICATIONS

1.1 The equipment must be random access, discrete, fully Automated Clinical Biochemistry/

Chemistry analyzer with back up equipment of the same make and model. Both equipment

must be brand new.

1.2 The equipment should work on photometric principle and Ion Selective Electrode (ISE)

1.3 Exact quoted model must be US-FDA or CE certified.

1.4 Equipment shall have an approximate throughput of 600 tests per hour for photometric analysis.

(Refer point no. 2.5 to 2.7)

1.5 Equipment must have STAT (Short Turn Around Time) facility to perform emergency test

1.8 If the following tests are not available in the existing test repertoire of the equipment, the

company or selected bidder shall set up the program on the equipment using third party

standard reagents. The equipment shall be compatible to perform these parameters

satisfactorily within approved quality specification criteria.

The company or selected bidder shall arrange and supply the required kits, reagents,

calibrators, controls-preferably third party and any other materials necessary to perform

these parameters and ensure that the tests are carried out properly.

1.9 Both equipment must have bidirectional interfacing facility and company / selected bidder

should support for interfacing with user’s information system / Gujarat Hospital Management

information system / NextGen eHospital. The bidirectional interfacing facility of both

equipment units shall be integrated and made fully functional with the existing laboratory

information system. All applicable charges for the same shall be borne by the company or

selected bidder.

1.10 To analyze the IQC data and LJ chart plotting, the company / selected bidder shall provide

Unity QC software from Bio-Rad or interlaboratory data management software from Randox

at no additional cost.

1.11 R.O. water system shall be provided and maintained by the company or selected bidder, if

required for the operation of equipment.

1.12 Both equipment must be provided with UPS unit to run system, minimum for 2 hours in case

of electricity failure.

1.13 The exact model of the quoted equipment shall have a minimum of two installations in NABL

accredited laboratories anywhere in India. The equipment shall be included under the Clinical

Biochemistry scope of NABL accreditation with at least 15 analytes from the test repertoire

mentioned below.

Documentary evidence for the same shall be provided by the company or selected bidder.

2. PERFORMANCE SPECIFICATIONS (TO BE DEMONSTRATED)

Equipment: Exact model of quoted equipment must be demonstrated for verification of

throughput and quality as follows

2.1 Venue: The demonstration may be conducted at any location, including a working

laboratory, the company’s operational site, or Civil Hospital Gandhinagar. The cost shall be

borne by the company or selected bidder.

2.2 Persons (To Whom): To technical experts and biomedical engineer and / or as assigned by

Hospital Medical superintendent.

2.3 Tests (Analytes) to be performed (Total 07):

Alanine aminotransferase (ALT), Albumin, Bilirubin Total, Calcium Total, Creatinine,

Glucose and Potassium

The company or selected bidder shall bring all required reagents, calibrators, third party

assayed internal quality control materials (2 levels) of their choice, either Bio Rad or

Randox, along with all consumables needed to perform the above mentioned tests

(analytes). No additional cost shall be charged at the time of demonstration.

The materials provided shall be sufficient to perform each test 40 times, comprising 20 runs

using normal range quality control samples and 20 runs using pathological range quality

control samples.

The equipment shall be in proper working condition and fully operational at the time of

All the tests listed above shall be calibrated during the demonstration using fresh reagent

kits and calibrators.

Pathological and normal range human sera based assayed internal quality control material

from a third party shall be run for each of the above mentioned analytes, excluding

potassium. Both levels of 06 analytes shall be tested 20 times.

Five minutes after the first result is obtained, two STAT samples (Total Bilirubin test) shall

be requested to demonstrate the STAT function. These samples may be quality control sera

or patient samples.

Pathological and normal range human sera based assayed internal quality control samples

from a third party shall be run for the potassium test. Each level shall be tested 20 times.

2.6 Evaluation:

For throughput: Only the 1st batch time shall be considered for throughput calculation.

Starting time: The starting time shall be recorded using the earliest observable event

available on the analyzer, in the following order of preference:

1) If the first sample aspiration is visible or the analyzer displays or records the first

sample aspiration time, that time shall be considered the starting time.

2) If the first sample aspiration is not observable and the analyzer does not display or

record the aspiration time, the time of loading the first sample onto the analyzer shall be

considered the starting time.

3) If neither the first sample aspiration time nor the sample loading time can be observed

or recorded, the starting time shall be the time at which the operator initiates the

analytical run by selecting the "Start", "Run", or equivalent function in the analyzer

End time: When last result of 1st batch is displayed [with command of 06 (Six) analytes run

for 20 + 20 times, means [(06 x 20) + (06 x 20) = 240 tests, 240 + 02 (STAT tests)] =

tests results, that time will be considered as end time.

For Analytical goals: Results for all analytes shall be recorded. Mean, SD, CV %, Bias and

Bias % for each analyte will be calculated.

material shall be selected in the following order of preference:

1) Instrument and method specific derived peer group mean obtained from the

assayed quality control manufacturer's real world / real time peer group data.

3. REQUIREMENT AFTER INSTALLATION OF EQUIPMENT

Company or selected bidder has to provide Installation Qualification (IQ), Operational

Qualification (OQ) and Performance Qualification (PQ) certificates and raw data, with no

additional cost.

For Method verification (Precision, Accuracy, linearity): Company has to provide

calibrators, reagents and IQC material for each test mentioned in commercial bid, with no

additional cost for method verification as per CLSI EP15-A3 guideline at the installation site.

4. REQUIREMENT TO ENSURE QUALITY DURING DAY TO DAY USE

Internal Quality Control (IQC): The company or selected bidder shall provide third party

assayed internal quality control material (e.g. Bio-Rad or Randox), at least two levels, in

sufficient quantity to run controls as per the latest NABL guidelines for each parameter

mentioned in the commercial bid, at no additional cost. The IQC material must have a long

expiry date, with a minimum of 18 months remaining at the time of receipt at the

QC software to get real time / real world data from respective manufacturer of assayed

internal quality control material at no additional cost.

External Quality Control Assessment (EQA) / Proficiency Testing (PT) Program: The

Laboratory, GMERS Medical College and attached General Hospital, Gandhinagar, in a

Proficiency Testing or EQA program [Either in Bio-Rad, Randox, CAP - College of American

Pathology or any other Proficiency Testing or EQA program providers, as per ISO 17043] for

each parameter mentioned in the commercial bid, at no additional cost.

IQC and EQA material has to be provided with no additional cost. However, tests of IQC and

EQA will be counted and chargeable under cost-per-reportable test.

4.1 Equipment calibration:

Equipment calibration shall be performed as per the manufacturer’s guidelines and

calibration certificates shall be provided to the user.

4.2 Calibration for the Analytes:

Calibrators for each parameter shall be traceable to a reference method or reference

material, as applicable. A traceability certificate shall be provided with each new lot

5. TERMS AND CONDITIONS FOR DOWNTIME PENALTY (EQUIPMENT BREAK-DOWN

The company or selected bidder shall provide urgent service and maintenance in case of

equipment breakdown to ensure that the clinical biochemistry laboratory remains

operational on a 24 × 7 basis throughout the year.

In the event of any breach in laboratory service provision to hospital patients, a penalty

shall be deducted from the monthly payment bill.

Calculation of time for penalty:

Time A shall be the time of SMS or phone call or Email made to the company, company

representative, or service engineer when any of the following conditions occur.

 The equipment is not functioning.

 Internal quality control shows violation of 22S, 13S, or any other Westgard rules that

are not resolved by the laboratory through routine root cause analysis.

 Reagents, calibrators, controls, consumables, or spares are not available.

 Any other situation in which the inability to report a test is traceable to the

equipment, its spares or consumables.

Time B shall be the time of confirmation of corrective action and successful verification

through internal quality control.

7. SUPPLY CONDITION:

All equipment, accessories, and related products shall be delivered to the workplace free of

cost. The company or selected bidder shall clearly mention the quantity supplied and

complete all applicable documentation and records as required by the hospital and

concerned department. The company shall maintain all required consumables, reagents,

calibrators, and controls as per requirement, with a minimum one month inventory to

ensure uninterrupted testing.

Goods shall be accepted only if supplied with proper cold chain maintenance and if they

meet acceptable receiving criteria at the Clinical Biochemistry Laboratory or hospital store.

8. MONTHLY TEST REPORT:

An authorized company or selected bidder representative shall generate a monthly report

of tests performed. The report shall include, analyte wise, the total number of tests

(requested tests, repeat tests, re-do tests, revised tests, auto or manual dilution tests,

internal quality control tests, and external quality assessment tests) performed during the

previous month.

The report shall be submitted between the 1st and 5th day of each month. It shall be

counterchecked and signed by an authorized person from the Department of Biochemistry.

Supporting data, including equipment printouts or soft copies of test records, shall be

attached to the report for verification of the actual number of tests performed.

Tests used for calibration of analytes shall not be counted or charged under the cost per

reportable test.

Delay in payment shall not be cited as a reason for non-supply or delay in supply of goods

contract by giving one month’s written notice, or a shorter period if mutually agreed, to the

contract holder (company or selected bidder). Upon expiry of the notice period, the contract

shall stand terminated without prejudice to any rights accrued up to the date of

11. RESPONSIBILITIES OF HOSPITAL:

The user shall provide appropriate air conditioned space, electricity, tap water, and

competent human resources for installation and operation of the equipment. Patient sample

collection, test analysis, daily quality control runs, routine daily equipment maintenance,

and generation of printout of reports shall be carried out by hospital personnel.

Access to the hospital premises and the Clinical Biochemistry Laboratory shall be

granted to the authorized company personnel for smooth execution of the CPRT

12. TRAINING AND DOCUMENT TO BE PROVIDED:

The authorized representative of company or selected bidder shall provide the required

training for operation and maintenance of the equipment to all technical and teaching staff

at the time of installation and as and when required.

The company or selected bidder may assess the competence of technical staff after

training and shall issue a certificate of training.

The company or selected bidder shall provide the user manual and technical manual

in hard copy, soft copy or both. Daily, weekly and monthly maintenance log books in

hard copy shall also be provided.

The company or selected bidder shall provide the Material Safety Data Sheets

(MSDS), traceability certificates of all reference materials or methods, and IQ, OQ,

and PQ certificates with raw data. All such documents shall be provided at the time

of installation and as and when required by the hospital or laboratory for the NABL

accreditation process.

Additional Tender Condition for Installation

 In the event of relocation of equipment within the premises, the activity shall be carried

out exclusively by the Original Equipment Manufacturer (OEM) or an authorized service

provider/bidder.

 The vendor shall, at no additional cost, undertake the following:

 Safe dismantling, shifting, and reinstallation of the equipment.

 Requalification of the equipment, including:

 Installation Qualification (IQ)

 Operational Qualification (OQ)
