FULLY AUTOMATED IMMUNOLOGY CHEMILLUMINESCENCE

PAST PERFORMANCE: : Order will be placed on the manufacturer or its authorized agent having proven performance. The bidder (themselves or through reseller) should have supplied same or similar item (where similar item refers to a FULLY AUTOMATED IMMUNOLOGY CHEMILLUMINESCENCE / ELECTRO CHEMILUMINESCENCE ANALYZER) to SWR or any Central/State Government organization or Public Sector Undertaking (PSU) in the last three calendar years. Firm should submit documentary evidence as proof of execution of PO, such as copies of Receipt Notes/CRAC/CRN/completion report as proof of successful execution of Purchase Orders. Failure to furnish the requisite documentary evidence along with the offer shall be construed as lack of past performance, and the offer shall not be considered for placement of bulk order.

Compliance to Technical specification: : Firm participating in tender to give para wise compliance of specification attached along with tender. Offer submitted without compliance will not be considered for further evaluation

Consumables and Reagents: : Firm should quote rates for the reagents enclosed in documents as Annexure-II. The financial evaluation will be made by considering the Rates of reagents and the cost of the machine. NOTE: Cost per Reportable Test (CPRT) must include rate of reagent, consumable, calibrator, control and any other item required to perform the same parameters.

Participation of Authorized Dealer: : Procurement will be made from only original manufacturers or their authorized dealers with tender specific authorization. Manufacturers / Authorized dealers to submit documents in support of their claim. Any authorized dealer quoting must enclose tender specific authorization from their manufacturer failing which their offers will be summarily rejected.

Are you coming under MSE status given as per guidelines issued by Railway Board for purchase preference vide letter No. 2010- RS [G] -363-1 Dt. 05.07.2012 or latest.

If yes, have you attached valid documents towards being MSE?

Have you submitted authenticated copy of the document authorizing the signatory to submit offer and commit on behalf of tenderers. Other conditions will be as per attached tender document

Have you attached valid UDYAM certificate if applicable to you?

Have you quoted Mode of dispatch on FOR destination basis for free delivery at destination with rate structure clearly indicating the basic rate, GST and freight etc, up to destination [consignees premises]

Have you quoted the discount if any in the specified column only in IREPS?

Have you furnished Bank account details such as Name of the Bank and address, account no., MICR, IFSC code and type of account to facilitate EFT payment?

The bidder confirms that Rates and other financial terms quoted in relevant columns of financial bid will only be ruling terms for acceptance, and such terms quoted anywhere else should be ignored. [Denial of this condition not recommended]

Firms shall submit past performance particulars for the tendered item along with proof of documents such as PO/R Note/CRN etc.,
