Maintenance of Garden in BMCRC Group of Combined Hospitals and Colleges Ballar

GOVERNMENT OF KARNATAKA DEPARTMENT OF LABOUR REGISTRATION CERTIFICATE OF ESTABLISHMENT The Agency should have valid Registration Certificate of Establishment issued under Karnataka Shops and Commercial Establishment act of 1961.

EXPERIENCE CERTIFICATE With WORK ORDERS:- The Agency should have Experience of provided man power for Govt Hospitals / Government Autonomous Medical Institutions / should have provided Manpower for Maintenance of Garden details as per the EXPERIENCES CERTIFICATES as per ANNEXURE-IV or its equivalent or as per the principal Employers Format that consisting of required information and its related WORK ORDER AND WORK DONE CERTIFICATES should be uploaded of original / self-attested.

SELF DECLARATION :-The Agency shall give Self Declaration in Rs.500/- Stamp Paper attested by the Government Notary as per the ANNEXURE-II.

QUALITY CONTROL CERTIFICATE:- The Agency should have valid ISO certificate or its equivalent Quality Control Certificate for Man Power Services and should be uploaded in original.

TENDER OFFER FORM :-The Tender Offer Form as per the ANNEXURE -VII should be uploaded in original

PAN CARD:-The Agency or Tenderer should have PAN CARD and should be uploaded in original/self attested.

GST REG 06 CERTIFICATE:-The Agency should have GST REG-06 registration certificate and should be uploaded in original /self attested.

GST RETURNS STATEMENT:-Last 6 Months GST Paid Challans (Dec-2024 to May -2025) Should be submit and last 3 years 2022-23, 2023-24 & 2024-25 returns from Assistant Commissioner Commercial Tax. As per Annexure-IX

EPF REGISTRATION CERTIFICATE:-The Agency should have valid EPF registration certificate issued by the Office of the Regional Provident Fund Commissioner as per EPF Act and should be uploaded in original.

EPF Paid Challan Receipt : Last 06 Months ECR Challans (Dec-2024 to May-2025) should be submit and last 3 years 2022-23, 2023-24 & 2024-25 from Competent Authority Should be submit. Annexure-X

ESI REGISTRATION CERTIFICATE:-The Agency must have valid ESI registration certificate issued by the Office of Sub-Regional, ESIC or competent authority as per ESI Act and should be uploaded in original.

ESI Paid Challan Receipt:- Last 06 Months ESI Paid Challans (Dec-2024 to May-2025)should be submit and last 3 years 2022-23, 2023-24 & 2024-25 from Competent Authority Should be submit Annexure-XI

PROFESSIONAL TAX REGISTRATION FORM No.3 CERTIFICATE:- The Agency must have Certificate of Enrolment issued under Karnataka Tax on Professions, Traders, Calling and Employment Act1976 (Form-3) or its equivalent should be uploaded in original.

PROFESSIONAL Paid Challan Receipt:- Last 3 years 2022-23, 2023-24 & 2024-25 PT Clearance Certificate from Competent Authority Should be submitted.

ANNUAL TURNOVER CERTIFICATE:- The Average Annual financial turnover of the tenderer should be not less than estimated cost of the quantity in the Tender Document during the last 3 proceedings financial years (Financial Year 2022-23, 2023-24 and 2024-25) The CA certified turnover certificate with UDIN should be uploaded. ANNEXURE-VIII.

ITR RETURNS:-Income Tax Returns ITR for the last Three Financial years 2022-23, 2023-24 and 2024-25 in original should be uploaded duly certified by the -Chartered Accountant.

EMD:-The Tenderer should remit Earnest Money in the e-portal and Receipt is to be uploaded in original with seal & Signature
