Details of human resource services currently being provided by the tenderer to various Government / Semi-Government organizations during the current year, specifying the number of staff provided to each organization (Copies of work orders from the organizations must be attached).

Name and Address of the Tenderer (Including Regional / Branch Office Address)

Whether the Tenderer is an Individual or an Organization if an Organization, registration details along with copies must be attached.

Registration details with the Labour Department (Copy of the Registration Certificate must be attached).v

Service Tax Registration details (Copy of the certificate must be attached).

GST Number must be specified (Copy of the certificate must be attached).

Details of the Code Number obtained under the EPF Act (Copy to be attached).

Details of the Code Number obtained under the ESI Act (Copy to be attached).

Affidavit on Rs.100/- stamp paper stating that the firm/organization is not blacklisted.

Registration details regarding Professional Tax (Copy of the certificate must be attached).

Income Tax returns submitted for the last 3 years (2023-24, 2024-25, and 2025-26).

The tenderer must have an average annual turnover capacity of at least 50% of the total estimated tender value over the last 3 to 5 financial years (Audit report copy Only the signed Audit Abstract must be uploaded/attached).
