Satisfactorily completed, at least one similar Contract of collecting, transporting, treatment and disposal of sanitary or Bio medical waste of quantity not less than 4500 kg per month as prime contractor (Attach Experience Certificate issued by the Employer of State Government / Central Government /State or Central undertaking boards / Urban local bodies /Pvt or Govt Health care centers)

Achieved in at least three financial years in last five years (i.e. 2020-21 to 2024-25) annual financial turnover of Rs.15.77 lakhs. (Attach Certified Annual Turnover certificate issued by Charted accountant.

Firm registration certificate (Service provider needs to submit the firm registration certificate).

Authorisation Obtained From Pollution Control Board for collection, Transportation, treatment and disposal. Relevent documents to be submitted.

GST Registration Certificate : ( Attach valid GST Registration Certificate and Pan Card of agency obtained from the issuing authorities)

Line of Credit from a nationalised Bank

Anti Collusion Certificate and self-declaration certificate regarding not black listed by any firm/ Government agencies in the format set out in this document.
