A copy of Form C Licence Certificate registered with the Labour Department and FORM VI (issued for providing services to 5-10 staff) should be attached and should be valid.

Details of the code number obtained under the EPF Act (Attach a copy of the certificate and the payment slip for the years 2022-23, 2023-24 & 2024-25 e.g. if the tender amount is Rs. 10.00 lakh then Rs. 15.00 lakh or if it is Rs. 20.00 lakh then Rs. 30.00 lakh per year)

Details of the code number obtained under the ESI Act (Attach a copy of the certificate and a copy of the payment slip for the years 2022-23, 2023-24 & 2024-25 e.g. if the tender amount is Rs. 10.00 lakhs then Rs. 15.00 lakhs or if it is Rs. 20.00 lakhs then Rs. 30.00 lakhs with the slips for each year)

Professional Tax Registration Details (Certificate and 2023-24 & 2024-25 roll copy should be attached)

PAN Card Details: If the organization is an individual, then in the name of the individual and if it is an organization, then in the name of the organization, then a copy of the PAN card should be submitted and the income tax returns for the years 2022-23, 2023-24 & 2024-25 should be submitted along with a certified copy of Form 3 CB & 3 CD.

An affidavit on a notarized paper of Rs. 100/- dated after the date of issue of the tender notice stating that the firm is not blacklisted should be submitted.

A copy of the ISO 9001:2015 quality certificate must be submitted

Certificate of annual turnover of the organization (Ex: Rs.50.00 lakh if the tender amount is Rs.10.00 lakh or Rs.100.00 lakh if the tender amount is Rs.20.00 lakh)

Details of the average annual turnover of at least Rs. 10.00-15.00 lakhs for the provision of HR services by the tenderer during the years 2021-22, 2022-23 and 2023-24 (Copies of audit reports (3CB & 3CD) certified by a Chartered Accountant should be attached)

To show financial soundness bank solvency (30% of the amount tendered for e-tender) should be obtained and submitted from a nationalized bank in the name of Executive Officer Taluk Panchayat sirsi.
