Copy of the Self declaration in Rs. 100 /- bond paper should be taken in the name of District Health and Family Welfare Society, Haveri ie The firm is not in Black listed and any other issues. Bond Should be taken after tender call dated. Submit the Self Declaration letter ie AVLT/ GPS System facility available in bidding vehicle.
